Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that reduces gastric acid secretion. Esoprol MUPS 20 mg is a delayed-release tablet formulated as Multiple Unit Pellet System, which helps protect the active ingredient from stomach acid until it reaches the intestine for absorption.
Uses & Indications
Dosage
Adults
GERD: 20 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as needed. H. pylori eradication: 20 mg twice daily for 7-10 days (in combination).
Elderly
No dosage adjustment generally required, but caution is advised in severe hepatic impairment.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Caution with severe renal impairment due to limited experience.
How to Take
Take one tablet whole with water at least one hour before a meal. Do not chew or crush the tablet. For patients who have difficulty swallowing, the tablet can be dispersed in a glass of non-carbonated water or apple juice.
Mechanism of Action
Esomeprazole is a specific inhibitor of the gastric proton pump (H+/K+-ATPase) in the parietal cells. It accumulates in the acidic secretory canaliculi of parietal cells, where it is converted to the active form and irreversibly binds to and inhibits the proton pump, thereby blocking the final step of acid secretion.

Excretion
Approximately 80% excreted as metabolites in urine and 20% in feces.
Half life
Approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours. Bioavailability is about 64% after a single 40 mg dose and increases to 89% after repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Tacrolimus
Increased tacrolimus levels, monitor concentrations.
Clopidogrel
Reduced antiplatelet effect, consider alternative antiplatelet therapy.
Methotrexate
Increased methotrexate levels, monitor for toxicity.
Nelfinavir, Atazanavir
Decreased plasma levels, avoid concomitant use.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include drowsiness, tachycardia, headache, blurred vision, abdominal pain, nausea, and vomiting. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is unknown whether esomeprazole is excreted in human milk; caution is advised when administering to a nursing mother.
