Description
Overview of the medicine
Etform 500 mg Tablet contains Metformin Hydrochloride, an oral medication primarily used to treat type 2 diabetes. It helps to lower blood glucose levels by improving the body's response to insulin.
Uses & Indications
Dosage
Adults
Initial dose: 500 mg once or twice daily with meals. Dosage may be gradually increased to a maximum of 2500 mg-3000 mg daily in divided doses. Taken with or after meals to reduce gastrointestinal side effects.
Elderly
Lower initial dose recommended, usually 500 mg once daily. Renal function should be monitored regularly due to increased risk of lactic acidosis.
Renal_impairment
Contraindicated if eGFR < 30 mL/min/1.73m². Dose adjustment required if eGFR is 30-60 mL/min/1.73m².
How to Take
Take orally, with meals (breakfast and/or dinner) to minimize gastrointestinal discomfort. Swallow the tablet whole with a glass of water, do not crush or chew.
Mechanism of Action
Metformin decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.
Pharmacokinetics
Onset
Glucose-lowering effects observed within a few days; maximal effects within 1-2 weeks.
Excretion
Excreted unchanged in the urine; primarily via renal tubular secretion.
Half life
Plasma elimination half-life is approximately 6.2 hours.
Absorption
Oral bioavailability is 50-60%. Absorption is slow and incomplete.
Metabolism
Metformin is not metabolized in the liver.
Side Effects
Contraindications
- •Severe renal impairment (eGFR < 30 mL/min/1.73m²)
- •Acute or chronic metabolic acidosis, including diabetic ketoacidosis, with or without coma
- •Hypersensitivity to metformin or any component of the formulation
- •Severe dehydration, acute heart failure, severe infection, hepatic impairment
Drug Interactions
Alcohol
Increases the risk of lactic acidosis.
Cimetidine
Increases metformin plasma concentration.
Iodinated Contrast Agents
Temporarily discontinue metformin before or at the time of an iodinated contrast imaging procedure in patients with an eGFR between 30 and 60 mL/min/1.73 m2; in patients with a history of liver disease, alcoholism, or heart failure; or in patients who will be administered intra-arterial iodinated contrast. Re-evaluate eGFR 48 hours after the imaging procedure, and restart metformin if renal function is stable.
Carbonic Anhydrase Inhibitors (e.g., Topiramate)
May increase the risk of lactic acidosis.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include lactic acidosis, which is a medical emergency requiring immediate hospitalization. Management involves supportive measures and hemodialysis to remove metformin from the body.
Pregnancy & Lactation
Pregnancy: Limited data suggest no increased risk of major congenital malformations. However, insulin is generally preferred for blood glucose control during pregnancy. Lactation: Metformin is excreted in breast milk. Consult a doctor to weigh the benefits against potential risks to the infant.
