Overview of the medicine
Etoricoxib is a selective non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation in conditions such as osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute gouty arthritis.
Uses & Indications
Dosage
Adults
For Osteoarthritis: 30-60 mg once daily. For Rheumatoid Arthritis/Ankylosing Spondylitis: 60-90 mg once daily. For Acute Gout: 120 mg once daily for a maximum of 8 days. For Acute Pain: 90 mg or 120 mg once daily for short-term use.
Elderly
No specific dosage adjustment usually required, but use with caution due to increased risk of side effects.
Renal_impairment
Use with caution in mild to moderate renal impairment. Not recommended in severe renal impairment (creatinine clearance <30 mL/min).
How to Take
Taken orally, with or without food. To be swallowed whole with water.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins, mediators of pain and inflammation.

Onset
Pain relief typically begins within 30-60 minutes.
Excretion
Excreted primarily via urine (70%) and feces (30%) as metabolites.
Half life
Elimination half-life is approximately 22 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached in approximately 1 hour.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- •Active peptic ulceration or gastrointestinal bleeding
- •Severe congestive heart failure (NYHA Class II-IV)
- •Hypersensitivity to etoricoxib or any component of the tablet
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •Severe hepatic or renal impairment
Drug Interactions
Lithium
May increase plasma lithium levels.
Warfarin
May increase the anticoagulant effect of warfarin, increasing bleeding risk.
Diuretics
May reduce the natriuretic effect of diuretics.
Methotrexate
May increase methotrexate toxicity.
ACE inhibitors/ARBs
May reduce the antihypertensive effect of ACE inhibitors and ARBs.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Treatment involves symptomatic and supportive measures. Gastric lavage or activated charcoal may be considered if recent ingestion.
Pregnancy & Lactation
Not recommended during pregnancy, especially in the third trimester due to potential adverse effects on the fetus. Avoid during breastfeeding as it may pass into breast milk.
