Description
Overview of the medicine
Etoricoxib is a selective cyclooxygenase-2 (COX-2) inhibitor used for the treatment of pain and inflammation associated with various conditions like osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute gouty arthritis. It provides pain relief and reduces swelling.
Uses & Indications
Dosage
Adults
Acute Gouty Arthritis: 120 mg once daily. Use only for the acute symptomatic period, maximum 8 days. Acute Pain and Dysmenorrhea: 120 mg once daily. Use only for the acute symptomatic period, maximum 8 days. For other indications (OA, RA, AS), lower doses (30-90 mg) are usually recommended.
Elderly
No dosage adjustment generally necessary, but caution is advised due to increased risk of adverse effects in older patients. Start with the lowest effective dose.
Renal_impairment
Mild to moderate renal impairment (CrCl 30-80 mL/min): No dosage adjustment required. Severe renal impairment (CrCl <30 mL/min): Not recommended.
How to Take
Take orally, with or without food. Swallow the tablet whole with a glass of water. Do not crush or chew.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2), an enzyme responsible for the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever. By inhibiting COX-2, etoricoxib reduces the production of these inflammatory prostaglandins, thereby providing analgesic, anti-inflammatory, and antipyretic effects.

Within 24 minutes for acute pain, generally within 1 hour for significant effect.
Excretion
Excreted primarily via the kidneys (70%) and feces (20%) as metabolites.
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations (Tmax) reached in approximately 1 hour. Oral bioavailability is about 100%.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzymes (CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to etoricoxib or any other ingredient, or to other NSAIDs (e.g., aspirin)
- •Active peptic ulceration or gastrointestinal bleeding
- •Severe hepatic dysfunction
- •Severe renal dysfunction (CrCl <30 mL/min)
- •Inflammatory bowel disease
- •Congestive heart failure (NYHA Class II-IV)
- •Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- •Uncontrolled hypertension with blood pressure persistently above 140/90 mmHg
Drug Interactions
Digoxin
Increased plasma digoxin concentrations.
Lithium
Increased plasma lithium levels, increased lithium toxicity.
Warfarin
Increased prothrombin time/INR, increased risk of bleeding.
Diuretics
Reduced diuretic and antihypertensive effect.
Rifampicin
Decreased etoricoxib plasma concentrations.
Methotrexate
Increased methotrexate plasma concentrations, increased toxicity.
Oral Contraceptives
Increased plasma concentrations of ethinyl estradiol, potentially increasing adverse effects.
ACE Inhibitors / Angiotensin Receptor Blockers (ARBs)
Reduced antihypertensive effect, increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote for etoricoxib overdose. Close monitoring of vital signs and management of any emergent complications (e.g., gastrointestinal or renal effects) is essential.
Pregnancy & Lactation
Etoricoxib should be avoided during pregnancy, especially during the third trimester, due to potential risks to the fetus (e.g., premature closure of ductus arteriosus). It is not known if etoricoxib is excreted in human milk, so it should be avoided during breastfeeding.
