Description
Overview of the medicine
Eval 250 mg Tablet contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria. It is effective against respiratory tract infections, urinary tract infections, otitis media, pharyngitis, tonsillitis, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg daily, either as a single dose or in two divided doses (e.g., 200 mg every 12 hours). For severe infections, the dose may be increased. For uncomplicated gonorrhea, a single 400 mg dose is recommended.
Elderly
No specific dose adjustment is required solely based on age; however, renal function should be assessed and dose adjusted if renal impairment is present.
Renal_impairment
For creatinine clearance less than 60 mL/min, the dose should be reduced. For creatinine clearance 21-60 mL/min, 300 mg daily. For creatinine clearance less than 20 mL/min, 200 mg daily.
How to Take
Eval 250 mg Tablet should be taken orally with water. It can be taken with or without food. To achieve best results, complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
Mechanism of Action
Cefixime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This leads to the disruption of the cell wall, resulting in bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours after administration.
Excretion
Excreted predominantly by the kidneys (approximately 50% as unchanged drug in urine) and partially via biliary/fecal route.
Half life
Plasma half-life is typically 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food can delay absorption but does not significantly alter the extent of absorption.
Metabolism
Minimal hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase peak plasma levels and AUC of Cefixime.
Carbamazepine
Elevated carbamazepine levels have been reported when co-administered with Cefixime.
Anticoagulants (e.g., Warfarin)
May increase prothrombin time (PT) with or without bleeding, monitor INR closely.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; use with caution in nursing mothers.
