Description
Overview of the medicine
Evoxil-CV 500 mg tablet is a broad-spectrum antibiotic medication containing Amoxicillin and Clavulanic Acid. It is used to treat bacterial infections resistant to Amoxicillin alone, due to the action of beta-lactamase enzymes.
Uses & Indications
Dosage
Adults
One 500 mg/125 mg tablet every 8 hours, or one 875 mg/125 mg tablet every 12 hours, depending on the severity of infection. Duration usually 7-10 days.
Elderly
No specific dose adjustment is required unless there is severe renal impairment.
Renal_impairment
Dosage adjustment is necessary. For creatinine clearance 10-30 mL/min, 500 mg/125 mg every 12 hours. For <10 mL/min, 500 mg/125 mg every 24 hours. Hemodialysis patients require an additional dose during and after dialysis.
How to Take
Oral. Should be taken at the start of a meal to minimize potential gastrointestinal intolerance and enhance absorption of clavulanic acid.
Mechanism of Action
Amoxicillin, a beta-lactam antibiotic, inhibits bacterial cell wall synthesis. Clavulanic Acid is a beta-lactamase inhibitor that protects Amoxicillin from degradation by bacterial beta-lactamase enzymes, thus extending its spectrum of activity.
Pharmacokinetics
Onset
Typically within 1-2 hours.
Excretion
Primarily via renal excretion (glomerular filtration and tubular secretion) for both components.
Half life
Amoxicillin: 1-1.5 hours; Clavulanic Acid: 1 hour (longer in renal impairment)
Absorption
Both Amoxicillin and Clavulanic Acid are well absorbed from the gastrointestinal tract after oral administration. Absorption is not significantly affected by food.
Metabolism
Amoxicillin is partially metabolized; Clavulanic Acid is extensively metabolized.
Side Effects
Contraindications
- History of hypersensitivity to penicillin or other beta-lactam antibiotics
- History of Amoxicillin/Clavulanate associated jaundice/hepatic dysfunction
- Severe renal impairment (without dose adjustment)
Drug Interactions
Probenecid
Increases Amoxicillin blood levels by reducing renal tubular secretion.
Allopurinol
Increased risk of rash.
Methotrexate
Increases methotrexate toxicity.
Oral contraceptives
May reduce the efficacy of oral contraceptives (advise additional contraceptive methods).
Anticoagulants (e.g., Warfarin)
May prolong prothrombin time; monitor INR.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms may include gastrointestinal symptoms (abdominal pain, vomiting, diarrhea) and fluid and electrolyte imbalance. Rarely, crystalluria leading to renal failure has been observed. Treatment is symptomatic and supportive. Hemodialysis can remove both Amoxicillin and Clavulanic Acid from circulation.
Pregnancy & Lactation
Pregnancy Category B: Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Lactation: Both components are excreted into breast milk. Use with caution in nursing mothers, monitor for potential infant sensitization or gastrointestinal effects.

Lab Monitoring
- Liver function tests (LFTs): Periodically, especially in patients with pre-existing hepatic impairment or prolonged therapy.
- Renal function tests: In patients with renal impairment.
- Complete blood count (CBC): For prolonged therapy.
Doctor Notes
- Crucial to differentiate between true allergic reaction (anaphylaxis, angioedema) and non-allergic rash to guide future antibiotic choices.
- Monitor renal and hepatic function in at-risk patients, especially with prolonged therapy.
Patient Guidelines
- Complete the full course of medication even if symptoms improve.
- Take with food to reduce stomach upset.
- Report any severe diarrhea, rash, or yellowing of the skin/eyes to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Generally, this medicine does not impair ability to drive or operate machinery. However, if side effects like dizziness or headache occur, avoid such activities.
Lifestyle Advice
