Description
Overview of the medicine
Exib (Etoricoxib) is a selective cyclooxygenase-2 (COX-2) inhibitor, used for the symptomatic relief of various inflammatory conditions and pain.
Uses & Indications
Dosage
Adults
Osteoarthritis: 30 mg or 60 mg once daily. Rheumatoid Arthritis, Ankylosing Spondylitis: 90 mg once daily. Acute Gouty Arthritis: 120 mg once daily (max 8 days). Acute Pain/Dysmenorrhea/Dental Surgery: 60 mg or 90 mg once daily (max 3 days).
Elderly
No dose adjustment generally required, but caution due to potential age-related renal impairment.
Renal_impairment
Mild to moderate (CrCl 30-80 mL/min): No dose adjustment. Severe (CrCl <30 mL/min): Not recommended.
How to Take
Oral administration, with or without food. Can be taken with food to reduce gastrointestinal upset.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2), which is responsible for the synthesis of prostaglandins involved in pain, inflammation, and fever, while sparing COX-1, which is involved in gastric protection.
Pharmacokinetics
Onset
Onset of action within 24 minutes for acute pain, longer for chronic conditions.
Excretion
Excreted mainly via urine (70%) and feces (30%) as metabolites.
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached in approximately 1 hour.
Metabolism
Extensively metabolized, primarily via CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to etoricoxib or any component of the formulation.
- •Congestive heart failure (NYHA Class II-IV).
- •Ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease (including patients who have recently undergone coronary artery bypass graft (CABG) surgery or angioplasty).
- •Active peptic ulceration or gastrointestinal (GI) bleeding.
- •Severe hepatic dysfunction (Child-Pugh score >9).
- •Severe renal dysfunction (CrCl <30 mL/min).
- •Inflammatory bowel disease.
- •Pregnancy and lactation.
Drug Interactions
Lithium
Increased lithium plasma levels and toxicity.
Warfarin
Increased INR and bleeding risk.
Methotrexate
Increased methotrexate toxicity.
Oral contraceptives
Increased exposure to oral contraceptive hormones.
Cyclosporine, Tacrolimus
Increased nephrotoxicity risk.
Diuretics and ACE inhibitors
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive and symptomatic treatment should be initiated. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C in early pregnancy, Category D in the third trimester. Avoid during pregnancy and lactation due to potential risks to the fetus/infant and possible inhibition of prostaglandin synthesis.
