Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions like GERD, stomach ulcers, and to prevent NSAID-associated ulcers.
Uses & Indications
Dosage
Adults
For most conditions (e.g., GERD, erosive esophagitis): 40 mg once daily for 4 to 8 weeks. Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No specific dose adjustment is generally required based on age alone, but caution is advised due to potential for reduced liver function.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take orally, at least one hour before meals. Swallow the capsule whole with water; do not chew or crush it.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells. This inhibition prevents the final step of acid secretion, thereby reducing gastric acidity.

Onset
Within 1 hour.
Excretion
Mainly excreted in urine (approximately 80%) and feces (approximately 20%) as inactive metabolites.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 60% for a 40 mg dose.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole or any other benzimidazole substituted proton pump inhibitor.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased absorption and plasma concentrations of digoxin.
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma concentrations of methotrexate.
Atazanavir/Nelfinavir
Reduced absorption and plasma levels of these antiviral drugs.

Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

Symptoms of overdose are generally mild and self-limiting (e.g., gastrointestinal disturbances). Treatment is symptomatic and supportive. Esomeprazole is not readily dialyzable.

Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus. It is excreted in human milk; caution is advised when administered to a nursing mother.
