Description
Overview of the medicine
Exilok 40 mg Injection contains Esomeprazole, a proton pump inhibitor, used to reduce stomach acid production. It is prescribed for conditions like gastroesophageal reflux disease (GERD), ulcers, and other acid-related disorders, especially when oral therapy is not feasible.
Uses & Indications
Dosage
Adults
Usually 40 mg once daily by IV infusion over 10-30 minutes for 7-10 days, or as directed by physician. For Zollinger-Ellison syndrome, initial dose is 80 mg twice daily.
Elderly
No dosage adjustment generally required, but caution in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment; caution in severe impairment.
How to Take
Exilok 40 mg Injection is administered intravenously (IV) as an infusion over a period of 10-30 minutes. It should be reconstituted with sterile normal saline or 5% dextrose solution as per product instructions before use. Do not administer as a bolus injection.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This action effectively reduces both basal and stimulated gastric acid secretion, leading to prolonged acid suppression.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Mainly excreted renally (approximately 80%), with a smaller portion in feces (approximately 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after intravenous administration, high systemic bioavailability.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Clopidogrel
Reduced antiplatelet effect of clopidogrel (clinical significance debated, but caution advised).
Methotrexate
Possible increased methotrexate levels, especially with high-dose methotrexate.
Nelfinavir, Atazanavir
Reduced plasma levels of antiretroviral drugs, leading to loss of therapeutic effect.
Storage
Store below 30°C, protected from light and moisture. Do not freeze. Reconstituted solution should be used immediately or within specified time according to product leaflet.
Overdose
Symptoms may include gastrointestinal disturbances and weakness. Treatment is symptomatic and supportive. Esomeprazole is extensively protein bound and not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.

WHO Essential Medicine
YesClinical Trials
Numerous clinical trials have established the efficacy and safety of esomeprazole in various acid-related disorders including GERD, gastric and duodenal ulcers, and Zollinger-Ellison syndrome. Studies have compared its effectiveness to other PPIs and H2-receptor antagonists.
Lab Monitoring
- Magnesium levels (for long-term use)
- Liver function tests (in patients with hepatic impairment)
Doctor Notes
- Consider tapering dose for long-term PPI users to avoid rebound acid secretion upon discontinuation.
- Educate patients on potential long-term risks, especially for elderly patients or those with multiple comorbidities.
- Monitor magnesium levels periodically in patients on long-term therapy or concurrent diuretic use. Consider vitamin B12 supplementation for prolonged use.
Patient Guidelines
- Inform your doctor about all existing medical conditions and medications, especially if you are taking nelfinavir, atazanavir, warfarin, clopidogrel, or methotrexate.
- Report any unusual side effects or worsening of symptoms immediately.
- Do not stop treatment abruptly without consulting your doctor.
Missed Dose Advice
Since this is an injection administered by a healthcare professional, a missed dose is unlikely. If a dose is missed, inform your doctor immediately to reschedule.
Driving Precautions
Esomeprazole may cause dizziness or visual disturbances in some patients. If you experience these symptoms, avoid driving or operating machinery until you know how the medicine affects you.
Lifestyle Advice
- Avoid spicy, fatty foods, acidic beverages, and caffeine to reduce acid reflux.
- Maintain a healthy weight, quit smoking, and limit alcohol consumption.
- Elevate the head of your bed while sleeping to alleviate GERD symptoms.
