Description
Overview of the medicine
Empagliflozin (Exler) is an oral medication used to lower blood sugar in adults with type 2 diabetes mellitus and to reduce the risk of cardiovascular death in adults with type 2 diabetes and established cardiovascular disease. It is also approved for heart failure with reduced or preserved ejection fraction.
Uses & Indications
Dosage
Adults
Typically 10 mg once daily, may be increased to 25 mg once daily based on efficacy and tolerability.
Elderly
No dose adjustment generally needed based on age alone, but renal function should be monitored.
Renal_impairment
Not recommended for eGFR < 30 mL/min/1.73 m². Dose adjustment may be needed for eGFR < 45 mL/min/1.73 m².
How to Take
Take orally once daily in the morning, with or without food.
Mechanism of Action
Empagliflozin is a sodium-glucose co-transporter 2 (SGLT2) inhibitor. It reduces renal reabsorption of filtered glucose and lowers the renal threshold for glucose, thereby increasing urinary glucose excretion and reducing blood glucose levels.

Onset
Blood glucose lowering effects observed within hours.
Excretion
Excreted primarily via the urine (approx. 50%) and feces (approx. 50%).
Half life
Approximately 12.4 hours.
Absorption
Rapidly absorbed. Peak plasma concentrations reached within 1.5 hours.
Metabolism
Metabolized mainly by glucuronidation.
Side Effects
Contraindications

Drug Interactions
Diuretics
May increase risk of dehydration and hypotension.
Insulin or Insulin Secretagogues (e.g., sulfonylureas)
May increase risk of hypoglycemia.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Empagliflozin is unlikely to be removed by dialysis.
Pregnancy & Lactation
Not recommended during pregnancy due to potential fetal renal dysfunction. Use with caution during lactation; consider discontinuing nursing or the drug.
