Overview of the medicine
Exopan (Pantoprazole) is a proton pump inhibitor used to treat conditions involving excessive stomach acid, such as peptic ulcers, GERD, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD and erosive esophagitis: 40 mg once daily for up to 8 weeks. For duodenal or gastric ulcers: 40 mg once daily for 2-4 weeks. For H. pylori eradication: 40 mg twice daily for 7-14 days (in combination therapy). For Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No specific dose adjustment is generally required in the elderly unless significant renal or hepatic impairment exists. Use with caution.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
Hepatic_impairment
For severe hepatic impairment, a maximum dose of 20 mg once daily is recommended.
How to Take
Exopan tablets should be swallowed whole with water, not chewed or crushed. It is usually taken once daily in the morning, before breakfast. For twice-daily dosing, take one dose in the morning and one in the evening before meals.
Mechanism of Action
Pantoprazole is a proton pump inhibitor that irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This inhibition reduces gastric acid secretion.

Onset
Onset of action within 2.5 hours.
Excretion
Primarily excreted renally (approximately 80%), with a smaller proportion excreted in feces (approximately 20%).
Half life
Terminal elimination half-life is approximately 1 hour.
Absorption
Rapidly absorbed after oral administration. Bioavailability is about 77%. Highly protein-bound (approx. 98%).
Metabolism
Extensively metabolized in the liver via the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- •Co-administration with atazanavir or nelfinavir due to significantly reduced plasma concentrations.
Drug Interactions
Digoxin
Increased digoxin absorption due to altered gastric pH.
Warfarin
Increased INR and prothrombin time, monitor carefully.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased methotrexate levels, especially with high doses.
Atazanavir/Nelfinavir
Reduced absorption, avoid co-administration.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
No specific antidote for pantoprazole overdose is known. In cases of overdose, symptomatic and supportive treatment should be initiated. Pantoprazole is not significantly removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is excreted in human milk; use with caution in lactating women and consult a doctor.
