Overview of the medicine
Extracef 125 mg Pediatric Drop is an oral antibiotic suspension containing Cefixime, a third-generation cephalosporin. It is used to treat a variety of bacterial infections in children.
Uses & Indications
Dosage
Adults
Not typically used for adults in pediatric drop form. Adult dosage for Cefixime is generally 200-400 mg daily.
Elderly
Not typically used for elderly in pediatric drop form. Dosage adjustment may be needed based on renal function.
Renal_impairment
For children with creatinine clearance < 20 mL/min, the dosage should be halved. Consult a physician.
How to Take
For oral use. Reconstitute the powder with boiled and cooled water as directed by the pharmacist or physician. Shake the suspension well before each use. Can be taken with or without food.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby disrupting the synthesis of peptidoglycan, which is a vital component of the bacterial cell wall. This leads to the lysis and death of bacterial cells.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. The remainder is excreted via non-renal mechanisms, likely biliary/fecal.
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food can delay absorption but does not significantly affect the extent of absorption.
Metabolism
Not extensively metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to Cefixime or any other cephalosporin antibiotic.
- •History of severe hypersensitivity reaction to penicillin or any other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase cefixime peak plasma levels and AUC.
Carbamazepine
Elevated carbamazepine levels have been reported with concomitant cefixime administration.
Warfarin and other anticoagulants
May increase prothrombin time (INR) and risk of bleeding.
Storage
Store the dry powder below 30°C in a dry place, away from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and do not freeze.
Overdose
In case of overdose, supportive and symptomatic treatment is recommended. Gastric lavage may be indicated if performed shortly after ingestion. Hemodialysis or peritoneal dialysis is not substantially effective in removing Cefixime from the body.
Pregnancy & Lactation
Cefixime is Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
