Extracef-125 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections in children and adults. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Adults usually take higher strength tablets. For suspension, dosage is typically based on weight for children: 8 mg/kg/day, given as a single daily dose or in two divided doses.
Elderly
No specific dosage adjustment is required solely based on age. However, renal function should be assessed, and dose adjusted if renal impairment is present.
Renal_impairment
For creatinine clearance 21-60 mL/min, 75% of standard dose daily. For creatinine clearance <20 mL/min, 50% of standard dose daily.
How to Take
Administer orally, with or without food. Shake the bottle well before each use. Use the provided measuring device for accurate dosing.
Mechanism of Action
Cefixime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to lysis and death of bacterial cells.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 3-4 hours after administration.
Excretion
Primarily excreted unchanged in urine via glomerular filtration and tubular secretion (about 50%), and some biliary excretion.
Half life
About 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food does not significantly alter absorption.
Metabolism
Minimal metabolism; approximately 10% of the dose is metabolized.
Side Effects
Contraindications
- •Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •History of severe hypersensitivity reactions (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
Drug Interactions
Probenecid
May increase cefixime plasma levels by decreasing renal excretion.
Carbamazepine
Increased carbamazepine levels have been reported with concomitant cefixime administration.
Oral Anticoagulants (e.g., Warfarin)
May increase prothrombin time and INR; monitor coagulation parameters.
Storage
Store the dry powder below 30°C, protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard after the specified period.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Cefixime is excreted in breast milk in small quantities; use with caution during lactation.
