Overview of the medicine
Ezygo 10 mg Tablet contains Esomeprazole, a proton pump inhibitor (PPI) used to reduce stomach acid production. It is prescribed for the treatment of conditions like gastroesophageal reflux disease (GERD), peptic ulcers, and other conditions involving excessive stomach acid.
Uses & Indications
Dosage
Adults
The usual adult dose for mild to moderate GERD is 20 mg once daily. For symptomatic treatment of GERD (non-erosive), 20 mg once daily. For maintenance therapy, 10 mg once daily may be considered based on clinical response. For H. pylori eradication, usually 20 mg twice daily with appropriate antibiotics for 7-14 days.
Elderly
No specific dosage adjustment is required for elderly patients, but caution is advised due to potential for reduced hepatic function.
Renal_impairment
No dosage adjustment is required for patients with renal impairment. However, for severe renal impairment, caution is advised.
How to Take
Ezygo 10 mg tablet should be swallowed whole with water, at least one hour before a meal. Do not chew or crush the tablet.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase (proton pump) enzyme system in the secretory canaliculus of the gastric parietal cells. This inhibition prevents the final step of acid secretion, thereby reducing gastric acidity.
Pharmacokinetics
Onset
Within 1 hour of administration.
Excretion
Excreted primarily in urine (80%) and feces (20%) as metabolites.
Half life
Approximately 1 to 1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is about 64% after a single 40 mg dose and increases to 89% after repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Esomeprazole or any other benzimidazole or excipients.
- •Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Warfarin
Increased INR and prothrombin time; monitor closely.
Nelfinavir
Significant decrease in nelfinavir exposure; contraindicated.
Clopidogrel
Reduced antiplatelet efficacy of clopidogrel.
Methotrexate
Increased and prolonged serum levels of methotrexate.
Diazepam, Phenytoin
Decreased metabolism and increased levels of these drugs due to CYP2C19 inhibition.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Experience with Esomeprazole overdose is limited. Symptoms may include drowsiness, tachycardia, nausea, diarrhea, headache, and dry mouth. Management should be symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in human milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
