Overview of the medicine
Ezykof Plus syrup is a combination medicine used for the relief of cough, nasal congestion, and chest congestion associated with common cold, flu, and other respiratory infections. It contains an antitussive (Dextromethorphan), an expectorant (Guaifenesin), and a decongestant (Pseudoephedrine).
Uses & Indications
Dosage
Adults
10 ml (2 teaspoonfuls) every 4-6 hours, not exceeding 4 doses in 24 hours.
Elderly
Same as adult dose, but with caution due to potential sensitivity to decongestants.
Renal_impairment
Use with caution. Dosage reduction may be necessary. Consult a doctor.
How to Take
Oral administration. Use a measuring spoon or cup for accurate dosage. May be taken with or without food.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla to suppress cough. Guaifenesin works by thinning and loosening mucus in the airways, making it easier to cough up. Pseudoephedrine is a sympathomimetic amine that acts as a decongestant by vasoconstriction, reducing swelling and nasal congestion.

Onset
15-30 minutes for symptomatic relief.
Excretion
Primarily renally excreted (urine) as unchanged drug or metabolites.
Half life
Dextromethorphan: 1.4-3.9 hours; Guaifenesin: approx. 1 hour; Pseudoephedrine: 9-16 hours.
Absorption
Dextromethorphan, Guaifenesin, and Pseudoephedrine are well absorbed orally. Dextromethorphan undergoes rapid and extensive hepatic metabolism.
Metabolism
Dextromethorphan is metabolized by CYP2D6. Guaifenesin is rapidly metabolized to beta-(2-methoxyphenoxy) lactic acid. Pseudoephedrine is partially metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to any component of the syrup
- •Patients receiving monoamine oxidase inhibitors (MAOIs) or within 14 days of stopping MAOI therapy
- •Severe hypertension or severe coronary artery disease
- •Children under 2 years of age (Pseudoephedrine is generally not recommended in infants)
Drug Interactions
MAOIs
Concomitant use with MAOIs can cause hypertensive crisis (due to pseudoephedrine).
Beta-blockers
May reduce the antihypertensive effect of beta-blockers.
Alcohol/CNS depressants
May enhance the sedative effects of dextromethorphan.
Tricyclic antidepressants
May potentiate the pressor effects of pseudoephedrine.
Storage
Store below 30°C in a cool, dry place, away from direct sunlight. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe drowsiness, dizziness, nausea, vomiting, rapid heart rate, hypertension, hallucinations, and seizures. In case of overdose, seek immediate medical attention. Management is supportive and symptomatic.

Consult your doctor before use during pregnancy and lactation. This medicine should only be used if the potential benefit justifies the potential risk to the fetus or infant.
