Description
Overview of the medicine
Fexofenadine is a non-sedating antihistamine used to relieve symptoms associated with seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). It helps reduce sneezing, runny nose, itchy eyes/nose/throat, and skin itching and rash.
Uses & Indications
Dosage
Adults
For Seasonal Allergic Rhinitis: 120 mg orally once daily. For Chronic Idiopathic Urticaria: 180 mg orally once daily.
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal function. Monitor renal function.
Renal_impairment
For adults with impaired renal function (creatinine clearance < 80 mL/min), the recommended starting dose is 60 mg once daily.
How to Take
Take Fastel 120 mg Tablet orally with water, preferably before a meal. Do not take with fruit juices (like grapefruit, orange, or apple juice) as they may reduce absorption.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It competitively binds to H1-histamine receptors, preventing histamine from binding and causing allergic symptoms, without significant central nervous system penetration.
Pharmacokinetics
Onset
Onset of action is typically within 1 hour.
Excretion
Primarily excreted unchanged in the feces (approximately 80%) and urine (approximately 10%).
Half life
The elimination half-life is approximately 11 to 15 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations (Cmax) are reached approximately 1-3 hours after a dose.
Metabolism
Minimally metabolized; approximately 5% of the dose is metabolized, primarily by the liver.
Side Effects
Contraindications
- •Hypersensitivity to fexofenadine hydrochloride or any of the excipients.
- •Children under 12 years of age for this specific strength and indication (consult physician for appropriate pediatric formulations).
Drug Interactions
Erythromycin or Ketoconazole
These medications have been shown to increase plasma levels of fexofenadine. While generally not clinically significant, caution is advised.
Antacids containing aluminum or magnesium
Concomitant administration with aluminum- or magnesium-containing antacids may reduce the absorption of fexofenadine. Separate administration by at least 2 hours.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include dizziness, drowsiness, and dry mouth. Management should be symptomatic and supportive. Hemodialysis does not effectively remove fexofenadine from the blood.
Pregnancy & Lactation
Pregnancy: Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Lactation: Fexofenadine is excreted in human breast milk. Caution should be exercised when administered to a nursing woman.
