Description
Overview of the medicine
Fastel 180 mg tablet is an antihistamine medication used to relieve symptoms of seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives) in adults and children 12 years and older.
Uses & Indications
Dosage
Adults
180 mg once daily.
Elderly
No dose adjustment is generally required for elderly patients, unless renal impairment is present. In such cases, dosage should be adjusted as for renal impairment.
Renal_impairment
For patients with impaired renal function, the recommended starting dose is 60 mg once daily.
How to Take
Take orally, with or without food. Do not take with fruit juices (e.g., grapefruit, orange, apple juice) as they may reduce the absorption of fexofenadine.
Mechanism of Action
Fexofenadine is a selective peripheral H1-blocker. It inhibits histamine release from mast cells and antagonizes the effects of histamine at H1-receptors, thereby reducing allergic symptoms without causing significant central nervous system effects.
Pharmacokinetics
Onset
Onset of action within 1 hour.
Excretion
Primarily excreted unchanged in the feces (about 80%) and urine (about 10%).
Half life
Approximately 11-15 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached in approximately 1-3 hours.
Metabolism
Undergoes minimal hepatic metabolism (about 5%).
Side Effects
Contraindications
- •Hypersensitivity to fexofenadine or any components of the formulation.
Drug Interactions
Erythromycin and Ketoconazole
May increase fexofenadine plasma concentrations, though this is not consistently associated with increased adverse effects.
Aluminum and magnesium-containing antacids
Can decrease the absorption of fexofenadine. Separate administration by at least 2 hours.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, dizziness, and dry mouth. Management involves symptomatic and supportive treatment. Hemodialysis does not effectively remove fexofenadine from blood.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit outweighs the potential risk to the fetus. It is unknown if fexofenadine is excreted in human milk; use with caution during lactation.
