Overview of the medicine
Feluric 40 mg Tablet contains Febuxostat, a xanthine oxidase inhibitor, used to lower uric acid levels in patients with chronic gout or hyperuricemia.
Uses & Indications
Dosage
Adults
Initial dose is 40 mg once daily. If serum uric acid is >6 mg/dL after 2 weeks, increase to 80 mg once daily. Maximum dose is 80 mg once daily.
Elderly
No dose adjustment required for elderly patients.
Renal_impairment
Mild to moderate renal impairment: No dose adjustment. Severe renal impairment: Limited data, caution advised, consider 40 mg once daily.
How to Take
Feluric 40 mg tablet should be taken orally once daily, with or without food.
Mechanism of Action
Febuxostat works by selectively inhibiting xanthine oxidase, an enzyme responsible for converting purines into uric acid. By inhibiting this enzyme, Febuxostat reduces the production of uric acid, thereby lowering its concentration in the blood.
Pharmacokinetics
Onset
Uric acid lowering effect begins within 2 weeks.
Excretion
Approximately 49% excreted renally (8% as unchanged drug) and 45% fecally (12% as unchanged drug).
Half life
Approximately 5-9 hours.
Absorption
Well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 1-1.5 hours.
Metabolism
Extensively metabolized in the liver, primarily by oxidation via CYP1A2, CYP2C8, and CYP2C9, and conjugation via UDP-glucuronosyltransferase (UGT) enzymes.
Side Effects
Contraindications
- •Hypersensitivity to febuxostat or any component of the formulation
- •Concomitant use with azathioprine or mercaptopurine
Drug Interactions
Warfarin
Although no significant interaction found in studies, caution and INR monitoring are advised due to potential for altered metabolism.
Theophylline
Febuxostat may increase theophylline levels; monitor closely.
Azathioprine, Mercaptopurine
Febuxostat significantly increases plasma concentrations of these drugs due to xanthine oxidase inhibition, leading to severe toxicity. Concomitant use is contraindicated.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive and symptomatic treatment should be provided. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. It is unknown if febuxostat is excreted in human milk; caution is advised during breastfeeding.
