Description
Overview of the medicine
Femoston is a hormone replacement therapy (HRT) medicine containing two types of female hormones, an estrogen (estradiol) and a progestogen (dydrogesterone). It is used to relieve symptoms occurring after menopause and to prevent osteoporosis in postmenopausal women.
Uses & Indications
Dosage
Adults
Typically one tablet daily, taken continuously or sequentially depending on the formulation (e.g., Femoston 1/10 or 2/10 for sequential, Femoston-conti for continuous). Start with lowest effective dose.
Elderly
No specific dose adjustments required based on age, but caution is advised in very elderly patients due to increased risk of venous thromboembolism (VTE) and stroke.

How to Take

Mechanism of Action

Estradiol is readily absorbed from the GI tract; dydrogesterone is also rapidly absorbed with peak plasma concentrations occurring between 0.5 and 2.5 hours.
Metabolism
Estradiol extensively metabolized in liver to inactive compounds; dydrogesterone extensively metabolized to 20α-dihydrodydrogesterone (DHD), its main active metabolite.
Side Effects
Contraindications
- •Known, past, or suspected breast cancer
- •Known or suspected estrogen-dependent malignant tumours (e.g., endometrial cancer)
- •Undiagnosed vaginal bleeding
- •Untreated endometrial hyperplasia
- •Previous idiopathic or current venous thromboembolism (DVT, PE)
- •Active or recent arterial thromboembolic disease (e.g., angina, myocardial infarction, stroke)
- •Acute liver disease, or a history of liver disease where liver function tests have failed to return to normal
- •Porphyria
- •Hypersensitivity to the active substances or to any of the excipients
Drug Interactions
Warfarin
May alter anticoagulant effects; careful monitoring of INR is required.
Ritonavir, nelfinavir
May induce or inhibit estrogen/progestogen metabolism, altering drug levels.
Thyroid hormone replacement
HRT may increase thyroid-binding globulin, requiring increased thyroid hormone dose in some patients.
Enzyme inducers (e.g., rifampicin, phenytoin, carbamazepine, phenobarbital, St. John's Wort)
May reduce estrogen and progestogen levels, leading to decreased efficacy and potential uterine bleeding.
Storage

Overdose
Symptoms of overdose include nausea, vomiting, breast tenderness, dizziness, abdominal pain, drowsiness/fatigue, and withdrawal bleeding. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Femoston is contraindicated during pregnancy and lactation. If pregnancy occurs, treatment should be discontinued immediately.
