Description
Overview of the medicine
Ferocom is an iron and folic acid supplement used to treat and prevent various types of anemia, especially during pregnancy and lactation. It provides elemental iron in a well-tolerated form and essential folic acid for red blood cell formation.
Uses & Indications
Dosage
Adults
Usually 1 capsule daily after food, or as directed by a physician.
Elderly
Same as adult dosage, but caution may be needed in patients with pre-existing conditions.
Renal_impairment
Generally well-tolerated, but monitoring is advised. Dosage adjustment may be necessary in severe cases.
How to Take
Ferocom capsule should be taken orally, preferably after a meal to minimize gastrointestinal discomfort. Do not chew or crush the capsule.
Mechanism of Action
Iron is an essential component of hemoglobin, myoglobin, and several enzymes. It is vital for oxygen transport and cellular respiration. Folic acid is essential for DNA synthesis, cell division, and erythropoiesis (red blood cell formation). The combination helps correct iron deficiency and megaloblastic anemia.
Pharmacokinetics
Onset
Therapeutic effects are typically observed over several weeks to months as hemoglobin levels gradually normalize.
Excretion
Iron is mainly excreted in feces. Folic acid is excreted primarily in the urine, partly as metabolites and partly unchanged.
Half life
Iron does not have a true half-life; it is stored and reused. Folic acid has a plasma half-life of about 3 hours.
Absorption
Iron polymaltose complex is absorbed via a controlled mechanism, leading to good absorption with less gastrointestinal irritation compared to ferrous salts. Folic acid is well-absorbed from the gastrointestinal tract.
Metabolism
Iron is primarily stored in the reticuloendothelial system. Folic acid is metabolized in the liver to its active forms (e.g., tetrahydrofolate).
Side Effects
Contraindications
- •Hypersensitivity to any component of the formulation
- •Hemochromatosis (iron overload)
- •Hemosiderosis
- •Non-iron deficiency anemias (e.g., aplastic anemia, hemolytic anemia) unless iron deficiency is also present
Drug Interactions
Antacids
Can reduce iron absorption. Should be taken at least 2 hours apart.
Levothyroxine
Iron can reduce levothyroxine absorption. Separate administration by at least 4 hours.
Certain Antiepileptics
Folic acid can interact with some antiepileptics (e.g., phenytoin, phenobarbital), potentially reducing their effectiveness.
Tetracyclines/Quinolones
Iron can impair absorption of these antibiotics. Administer several hours apart.
Storage
Store in a cool, dry place, away from direct sunlight and moisture, below 30°C. Keep out of reach of children.
Overdose
Symptoms of iron overdose can include severe gastrointestinal upset, metabolic acidosis, shock, and potentially liver damage. Folic acid overdose is generally considered non-toxic. In case of suspected overdose, seek immediate medical attention. Management may involve gastric lavage and chelation therapy for iron toxicity.
Pregnancy & Lactation
Ferocom is frequently prescribed during pregnancy and lactation to prevent and treat iron and folate deficiencies, which are common during these periods. It is generally considered safe when used at recommended doses under medical supervision.
