Description
Overview of the medicine
Feroplus TR is a sustained-release capsule containing Ferrous Fumarate and Folic Acid, used to treat and prevent various types of anemia, especially iron deficiency anemia and megaloblastic anemia, particularly during pregnancy and lactation.
Uses & Indications
Dosage
Adults
Typically, one capsule daily or as directed by the physician. It should be taken preferably after meals to minimize gastrointestinal upset.
Elderly
Same as adults, but caution should be exercised in patients with pre-existing conditions or those on multiple medications.
Renal_impairment
Use with caution. Dosage adjustment may be required in severe renal impairment, though iron and folic acid are largely not renally cleared.
How to Take
Take the capsule orally, preferably after a meal, with a glass of water. Do not crush or chew the sustained-release capsule.
Mechanism of Action
Ferrous Fumarate provides elemental iron, which is crucial for hemoglobin synthesis and oxygen transport. Folic Acid is essential for DNA synthesis, cell division, and red blood cell maturation, preventing megaloblastic anemia.
Pharmacokinetics
Onset
Therapeutic effects are usually observed after several weeks of continuous treatment.
Excretion
Iron is minimally excreted; most is conserved. Folic acid is excreted primarily via urine, with some biliary excretion.
Half life
Elemental iron has a variable half-life depending on body needs. Folic acid has a plasma half-life of several hours.
Absorption
Ferrous Fumarate is absorbed primarily in the duodenum and upper jejunum, with absorption enhanced by gastric acid and Vitamin C. Folic Acid is absorbed mainly in the small intestine. 'TR' formulation ensures slow and sustained release.
Metabolism
Iron is not metabolized but is incorporated into various body compounds. Folic acid is metabolized in the liver to its active form, tetrahydrofolate.
Side Effects
Contraindications
- •Known hypersensitivity to Ferrous Fumarate, Folic Acid, or any excipients.
- •Iron overload conditions (e.g., hemochromatosis, hemosiderosis).
- •Patients receiving repeated blood transfusions.
- •Non-iron deficiency anemias (e.g., hemolytic anemia unless iron deficiency also exists).
Drug Interactions
Antacids
Reduced iron absorption. Take iron 2 hours before or 4 hours after antacids.
Methotrexate
Folic acid can interfere with the antifolate effects of methotrexate, especially in high doses.
Thyroid hormones
Reduced absorption of thyroid hormones. Separate by several hours.
Levodopa/Methyldopa
Iron may reduce absorption of these drugs.
Tetracyclines/Quinolones
Reduced absorption of both iron and antibiotics. Separate administration by at least 2-4 hours.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep away from direct sunlight.
Overdose
Symptoms of iron overdose include severe gastrointestinal upset (nausea, vomiting, abdominal pain, diarrhea), followed by pallor, cyanosis, shock, metabolic acidosis, and hepatic necrosis. Folic acid overdose is rare and generally considered non-toxic. Management: Gastric lavage, supportive care, and iron chelating agents (e.g., deferoxamine) for severe iron toxicity.
Pregnancy & Lactation
Feroplus TR is generally considered safe and often recommended during pregnancy and lactation to prevent and treat iron deficiency anemia and megaloblastic anemia. Consult a doctor before use.
