Description
Overview of the medicine
Ferovit-TR 150 mg capsule is an anti-anemic preparation containing sustained-release ferrous fumarate, folic acid, and ascorbic acid. It is primarily used for the treatment and prevention of iron deficiency anemia and megaloblastic anemia, particularly during pregnancy and lactation.
Uses & Indications
Dosage
Adults
Usually 1 capsule daily, or as directed by a physician. In severe cases, dosage may be adjusted.
Elderly
No specific dosage adjustment is usually required for elderly patients, but caution is advised in those with pre-existing conditions.
Renal_impairment
Use with caution in severe renal impairment. Consult a physician for appropriate dosing.
How to Take
Take orally, preferably with a meal or after food to minimize gastrointestinal upset. Do not chew or crush the capsule; swallow whole.
Mechanism of Action
Ferrous fumarate provides elemental iron, essential for hemoglobin synthesis and oxygen transport. Folic acid is crucial for DNA synthesis, cell division, and red blood cell maturation, helping to prevent megaloblastic anemia. Ascorbic acid (Vitamin C) enhances the absorption of non-heme iron from the gut.
Pharmacokinetics
Onset
Therapeutic effects, such as an increase in hemoglobin, are usually observed within 2-4 weeks of continuous treatment.
Excretion
Unabsorbed iron is excreted mainly in the feces. Folic acid and its metabolites are excreted primarily via the kidneys. Ascorbic acid and its metabolites are excreted in the urine.
Half life
Elemental iron has no specific half-life as it is primarily recycled in the body. Folic acid has an elimination half-life of about 3-4 hours. Ascorbic acid's half-life varies depending on dose and body stores, ranging from 10-20 days.
Absorption
Iron (from ferrous fumarate) is absorbed primarily in the duodenum and upper jejunum, enhanced by ascorbic acid. Folic acid is absorbed throughout the small intestine. Ascorbic acid is readily absorbed from the gastrointestinal tract. The sustained-release formulation allows for prolonged absorption and reduced gastrointestinal irritation.
Metabolism
Iron is not metabolized in the traditional sense; it is incorporated into hemoglobin or stored as ferritin/hemosiderin. Folic acid is metabolized in the liver to its active forms (e.g., tetrahydrofolate). Ascorbic acid is partially metabolized to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to any component of the formulation
- •Hemochromatosis or hemosiderosis (conditions of iron overload)
- •Anemias not caused by iron deficiency (e.g., aplastic anemia, hemolytic anemia, thalassaemia unless iron deficiency is also present)
- •Repeated blood transfusions
Drug Interactions
Phenytoin
Folic acid may decrease phenytoin levels. Monitor carefully.
Tetracyclines & Quinolones
Iron may reduce the absorption of these antibiotics. Administer iron 2-4 hours before or after these medications.
Levodopa, Methyldopa, Thyroxine
Iron may reduce the absorption of these drugs. Separate administration.
Antacids, Proton Pump Inhibitors, H2 Receptor Blockers
May reduce iron absorption. Separate administration by at least 2-4 hours.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of iron overdose may include nausea, vomiting, abdominal pain, diarrhea, dark stools, drowsiness, and in severe cases, circulatory collapse, metabolic acidosis, and hepatic necrosis. Seek immediate medical attention. Management involves gastric lavage, chelation therapy with deferoxamine.
Pregnancy & Lactation
Highly recommended and often prescribed during pregnancy and lactation to prevent and treat iron and folic acid deficiencies, which are common and crucial for maternal and fetal/infant health. Use under medical supervision.
