Description
Overview of the medicine
Ferovit-TR Capsule is a sustained-release iron and folic acid supplement designed to treat and prevent iron deficiency anemia and megaloblastic anemia, particularly during pregnancy, lactation, and other conditions requiring increased iron and folate intake. The sustained-release formulation helps to minimize gastrointestinal side effects.
Uses & Indications
Dosage
Adults
One capsule daily, preferably after a meal. In severe cases, dosage may be adjusted by a physician.
Elderly
No specific dosage adjustment required; use as per adult dosage or physician's advice.
Renal_impairment
Use with caution. Dosage may need adjustment based on severity and physician's advice.
How to Take
To be taken orally, preferably after a meal, with a full glass of water. Do not chew or crush the capsule.
Mechanism of Action
Ferrous fumarate provides elemental iron, an essential component of hemoglobin, myoglobin, and various enzymes, vital for oxygen transport and cellular respiration. Folic acid, a B vitamin, is converted to tetrahydrofolate, which is essential for erythropoiesis, DNA synthesis, and cell division, crucial for the maturation of red blood cells.
Pharmacokinetics
Onset
Improvement in iron deficiency symptoms can be observed within a few weeks; significant rise in hemoglobin occurs over several months. Folic acid effects on hematopoiesis can be seen within days for megaloblastic anemia.
Excretion
Iron is primarily conserved in the body with minimal excretion through feces, urine, and sweat. Folic acid and its metabolites are excreted mainly in the urine; excess orally administered folic acid is also excreted unchanged in the urine.
Half life
Iron has a complex physiological turnover rather than a typical half-life; excess iron is stored. Folic acid has a plasma half-life of several hours.
Absorption
Ferrous iron is absorbed primarily in the duodenum and upper jejunum; absorption is enhanced by sustained release formulation. Folic acid is absorbed rapidly from the gastrointestinal tract, primarily in the small intestine.
Metabolism
Iron is transported in plasma bound to transferrin and stored in the form of ferritin or hemosiderin. Folic acid is rapidly metabolized in the liver to tetrahydrofolate and other active forms.
Side Effects
Contraindications
- •Known hypersensitivity to ferrous fumarate, folic acid, or any component of the formulation.
- •Hemochromatosis and hemosiderosis (conditions involving iron overload).
- •Hemolytic anemia, aplastic anemia, or other anemias not related to iron or folate deficiency.
Drug Interactions
Antacids
Reduced iron absorption if taken concurrently. Separate by at least 2 hours.
Methotrexate
Folic acid can interfere with the antifolate effect of methotrexate.
Levothyroxine
Iron may reduce absorption of levothyroxine. Administer at least 4 hours apart.
Phenytoin/Other Anticonvulsants
Folic acid may decrease serum levels of some anticonvulsants.
Tetracyclines & Fluoroquinolones
Iron can chelate these antibiotics, reducing their absorption. Separate by 2-4 hours.
Storage
Store in a cool, dry place, below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of iron overdose may include severe gastrointestinal irritation (nausea, vomiting, diarrhea, abdominal pain), metabolic acidosis, circulatory collapse, and liver damage. Folic acid overdose is generally not toxic. In case of suspected overdose, seek immediate medical attention. Treatment involves supportive care, gastric lavage, and chelation therapy if iron levels are critically high.
Pregnancy & Lactation
Ferovit-TR is commonly prescribed and considered safe for use during pregnancy and lactation to prevent and treat iron and folate deficiencies. Consult a physician for appropriate dosage.
