Description
Overview of the medicine
Feva 500 mg Tablet contains Paracetamol, a widely used medication for the relief of mild to moderate pain and fever. It works by inhibiting prostaglandin synthesis in the central nervous system, reducing pain and fever.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg (1-2 tablets) every 4-6 hours as needed, not exceeding 4000 mg (8 tablets) in 24 hours.
Elderly
Same as adult dose; caution advised in patients with impaired renal or hepatic function.
Renal_impairment
Dose adjustment may be necessary in severe renal impairment. Consult a physician.
How to Take
Oral administration. Tablets should be swallowed with water. Can be taken with or without food. Do not exceed recommended dose.
Mechanism of Action
Paracetamol's exact mechanism is not fully understood, but it is believed to act primarily in the central nervous system by inhibiting prostaglandin synthesis through a COX-3 like mechanism, leading to analgesic and antipyretic effects. It has minimal anti-inflammatory activity.
Pharmacokinetics
Onset
30-60 minutes for analgesia; 30 minutes for antipyresis.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Approximately 1.5 to 3 hours; slightly prolonged in hepatic dysfunction or overdose.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, primarily in the small intestine. Peak plasma concentrations are reached within 30-60 minutes.
Metabolism
Primarily metabolized in the liver by conjugation with glucuronide and sulfate. A small portion is metabolized by cytochrome P450 into a highly reactive intermediate (NAPQI), which is normally detoxified by glutathione. In overdose, glutathione stores are depleted, leading to liver damage.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any component of the formulation
- Severe hepatic impairment or active liver disease
Drug Interactions
Alcohol
Increases risk of liver damage, especially with chronic heavy alcohol consumption.
Cholestyramine
Reduces paracetamol absorption.
Metoclopramide, Domperidone
Increases paracetamol absorption rate.
Warfarin and other coumarins
May increase anticoagulant effect, leading to increased bleeding risk with prolonged regular use.
Isoniazid, Rifampicin, Carbamazepine, Phenytoin
May increase risk of paracetamol hepatotoxicity by inducing liver enzymes.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, abdominal pain, sweating, and malaise, which may be followed by liver damage (jaundice, liver tenderness) in 24-48 hours. Seek immediate medical attention. Treatment involves gastric lavage, activated charcoal, and N-acetylcysteine (NAC) intravenously or orally.
Pregnancy & Lactation
Considered safe for use during pregnancy and lactation when used at recommended doses for short periods. It is the preferred analgesic/antipyretic in pregnancy. Consult a doctor before use.

- Renal function tests (creatinine) in patients with kidney impairment
Doctor Notes
- Emphasize strict adherence to dosage to prevent liver toxicity.
- Advise patients about potential for other OTC products containing paracetamol.
- Caution in patients with chronic liver disease, severe renal impairment, or chronic alcoholics.
