Description
Overview of the medicine
Fexofenadine is a non-sedating antihistamine used to relieve symptoms of seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives).
Uses & Indications
Dosage
Adults
Seasonal Allergic Rhinitis: 180 mg once daily. Chronic Idiopathic Urticaria: 180 mg once daily.
Elderly
No specific dosage adjustment is recommended, but caution is advised in elderly patients with impaired renal function.
Renal_impairment
For patients with impaired renal function (creatinine clearance ≤ 80 mL/min), initial dose should be 60 mg once daily.
How to Take
Take orally with water, preferably before a meal, as food may reduce absorption.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It blocks the effects of histamine by competitively inhibiting histamine H1-receptors on effector cells, thereby reducing allergic symptoms without causing significant central nervous system effects like sedation.

Onset
Onset of action is approximately 1 hour.
Excretion
Excreted primarily unchanged in feces (80%) and urine (11%).
Half life
Plasma elimination half-life is approximately 11-15 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached in 2-3 hours. Bioavailability is approximately 33%.
Metabolism
Minimally metabolized (approximately 5% by hepatic metabolism).

Contraindications
- Known hypersensitivity to fexofenadine or any excipients of the tablet.
Drug Interactions
Erythromycin or Ketoconazole
May increase plasma levels of fexofenadine, but generally without increasing side effects.
Fruit juices (grapefruit, orange, apple)
May reduce the bioavailability of fexofenadine. Avoid taking with these juices.
Antacids containing aluminum or magnesium
May reduce the absorption of fexofenadine. Separate administration by at least 2 hours.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose may include drowsiness, dizziness, and dry mouth. Treatment should be symptomatic and supportive, including gastric lavage and activated charcoal if appropriate.
