Description
Overview of the medicine
Filodox 100 mg Syrup contains Filoxol, a xanthine derivative bronchodilator. It is used to relax airway muscles, making it easier to breathe for patients with asthma, chronic obstructive pulmonary disease (COPD), and other reversible obstructive airway diseases.
Uses & Indications
Dosage
Adults
10 ml (200 mg) two to three times daily, preferably after meals.
Elderly
A reduced dose of 5-10 ml (100-200 mg) two times daily may be appropriate, considering potential reduced metabolism.
Renal_impairment
Use with caution. Dosage adjustment may be required based on clinical response and severity of impairment.
How to Take
Filodox Syrup should be taken orally, preferably after meals to minimize gastrointestinal upset. Use the provided measuring cup or spoon for accurate dosing. Shake well before use.
Mechanism of Action
Filoxol acts as a bronchodilator primarily by inhibiting phosphodiesterase enzymes, leading to an increase in intracellular cyclic AMP (cAMP) levels. This results in the relaxation of bronchial smooth muscles. It also has antagonistic effects on adenosine receptors.
Pharmacokinetics
Onset
Bronchodilation typically begins within 30-60 minutes after oral administration.
Excretion
Excreted mainly through the kidneys, with less than 10% of the unchanged drug appearing in urine.
Half life
The plasma half-life of Filoxol is approximately 6-8 hours, but can vary.
Absorption
Rapid oral absorption, reaching peak plasma concentrations within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP enzymes, into inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Filoxol, other xanthine derivatives, or any component of the formulation
- Acute myocardial infarction
- Severe cardiac arrhythmias
- Uncontrolled epilepsy
Drug Interactions
Cimetidine
May increase plasma concentrations of Filoxol, potentially leading to increased side effects. Dose adjustment may be necessary.
Erythromycin
Concurrent use can increase Filoxol levels, increasing risk of toxicity. Monitor closely.
Phenobarbital/Phenytoin
Can decrease plasma concentrations of Filoxol by inducing hepatic metabolism, reducing its efficacy. Higher doses of Filoxol may be needed.
Beta-adrenergic agonists
May have additive bronchodilatory effects and increased risk of cardiac side effects.
Storage
Store below 30°C (86°F) in a cool, dry place, protected from direct light and moisture. Do not freeze. Keep the bottle tightly closed when not in use. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, agitation, tremors, headache, dizziness, insomnia, and in severe cases, convulsions, and cardiac arrhythmias (e.g., tachycardia). Treatment is supportive and symptomatic. It may involve gastric lavage, administration of activated charcoal to reduce absorption, and management of seizures with benzodiazepines or arrhythmias with appropriate anti-arrhythmic drugs.
Pregnancy & Lactation
Pregnancy Category C. Studies in animals have shown adverse effects, but there are no adequate studies in pregnant women. Use only if the potential benefit justifies the potential risk to the fetus. Filoxol is excreted into breast milk. Caution should be exercised when administered to a nursing woman, or consider an alternative medicine.

Clinical Trials
Clinical trials have evaluated the safety and efficacy of Filoxol in patients with chronic obstructive airway diseases, demonstrating its ability to improve lung function parameters and reduce the frequency and severity of symptoms. Post-marketing surveillance continues to monitor real-world outcomes.
Lab Monitoring
- Plasma Filoxol levels (if therapeutic drug monitoring is available, especially in patients with impaired metabolism or potential interactions)
- Liver function tests (periodically in patients on long-term therapy or with pre-existing hepatic impairment)
- Renal function tests (periodically in patients with renal impairment)
Doctor Notes
- Educate patients on proper dosing technique using the provided measuring device.
- Advise patients to avoid concurrent use of other xanthine-containing products or excessive caffeine intake.
- Monitor for signs of toxicity (e.g., severe nausea, tremor, palpitations) and consider plasma level monitoring if available and indicated.
- Counsel on potential drug interactions, especially with macrolide antibiotics and cimetidine.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor, do not alter the dose or stop treatment without consultation.
- Use the provided measuring device to ensure accurate dosing.
- Report any new or worsening symptoms, or severe side effects to your doctor immediately.
- Avoid smoking, as it may decrease the effectiveness of Filoxol.
