Description
Overview of the medicine
Filodox 400 mg Tablet contains Doxofylline, a bronchodilator used for the treatment of bronchial asthma, chronic obstructive pulmonary disease (COPD), and chronic bronchitis. It helps to relax the muscles of the airways, making it easier to breathe.
Uses & Indications
Dosage
Adults
Usually 400 mg twice daily. In some cases, depending on severity and physician's advice, dosage may be adjusted.
Elderly
Lower doses may be considered, starting with 200 mg twice daily, with careful monitoring due to potential reduced clearance.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe renal impairment.
How to Take
Filodox 400 mg Tablet should be taken orally with or without food, as directed by your physician. Swallow the tablet whole with water; do not crush or chew it.
Mechanism of Action
Doxofylline is a xanthine derivative that primarily acts as a bronchodilator. It is believed to exert its action by inhibiting phosphodiesterase enzymes, leading to an increase in intracellular cyclic AMP (cAMP) levels. This results in the relaxation of bronchial smooth muscles, reducing airway obstruction. It has a better safety profile compared to traditional xanthines like theophylline.
Pharmacokinetics
Onset
Bronchodilatory effects usually begin within 30-60 minutes after oral administration.
Excretion
Excreted mainly through the urine as metabolites. Less than 4% is excreted unchanged.
Half life
Approximately 6-8 hours, allowing for twice-daily dosing.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver (more than 90%) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Doxofylline or other xanthine derivatives
- Acute myocardial infarction
- Severe cardiac arrhythmias
- Unstable angina pectoris
Drug Interactions
Phenytoin
May decrease Doxofylline effectiveness by inducing its metabolism.
Cimetidine
May inhibit Doxofylline metabolism, leading to increased plasma concentrations.
Propranolol
Concurrent use should be cautious as it may reduce the effectiveness of Doxofylline.
Erythromycin
May increase plasma levels of Doxofylline, increasing risk of adverse effects.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe nausea, vomiting, headache, restlessness, insomnia, palpitations, and in severe cases, cardiac arrhythmias or seizures. In case of overdose, seek immediate medical attention. Treatment is generally supportive, involving gastric lavage and symptomatic management.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit outweighs the potential risk to the fetus. Doxofylline is excreted into breast milk; therefore, caution should be exercised when administering to a nursing mother. Consult your doctor before use.

Doxofylline has undergone various clinical trials demonstrating its efficacy and safety in bronchial asthma and COPD. Studies have shown it to be effective with a better tolerability profile compared to theophylline.
Lab Monitoring
- Routine lab monitoring is not usually required. However, plasma doxofylline levels may be monitored in cases of suspected toxicity or altered pharmacokinetics, particularly in patients with hepatic or renal impairment.
Doctor Notes
- Advise patients on proper inhaler technique if they are also using inhaled bronchodilators.
- Educate patients about the importance of adherence to prescribed dosage and avoiding self-medication.
- Monitor for signs of cardiac arrhythmia, especially in patients with pre-existing heart conditions.
