Description
Overview of the medicine
Atorvastatin is used to lower cholesterol and triglycerides in the blood. It reduces the risk of heart disease and stroke by decreasing LDL-C (bad cholesterol) and increasing HDL-C (good cholesterol).
Uses & Indications
Dosage
Adults
Initial dose is usually 10-20 mg once daily. Max 80 mg once daily. For Finen 20 mg, the standard starting dose is 20 mg.
Elderly
No dose adjustment needed based on age alone.
Renal_impairment
No dose adjustment needed.
How to Take
Oral, once daily, with or without food. Can be taken at any time of day, but consistency is recommended.
Mechanism of Action
Selectively and competitively inhibits HMG-CoA reductase, an enzyme that controls the rate of cholesterol synthesis in the liver. This leads to a decrease in LDL-C and an increase in HDL-C.
Pharmacokinetics
Onset
Lipid-lowering effects typically observed within 2 weeks, maximal effect usually within 4 weeks.
Excretion
Primarily excreted in the bile following hepatic and/or extrahepatic metabolism. Less than 2% is recovered in urine.
Half life
Approximately 14 hours for atorvastatin and its active metabolites.
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations reached within 1-2 hours. Bioavailability is about 12-30%.
Metabolism
Primarily metabolized by cytochrome P450 3A4 (CYP3A4) to active ortho- and parahydroxylated metabolites.
Side Effects
Contraindications
- •Active liver disease or unexplained persistent elevations of serum transaminases.
- •Pregnancy and lactation.
- •Hypersensitivity to atorvastatin or any component of the formulation.
Drug Interactions
Warfarin
Increased anticoagulant effect in some patients.
Colchicine
Increased risk of myopathy/rhabdomyolysis.
Oral contraceptives
May increase concentrations of ethinyl estradiol and norethindrone.
Gemfibrozil, other fibrates, niacin
Increased risk of myopathy/rhabdomyolysis.
CYP3A4 inhibitors (e.g., cyclosporine, macrolide antibiotics, azole antifungals, protease inhibitors)
Increased atorvastatin plasma concentrations, increased risk of myopathy/rhabdomyolysis.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Symptomatic and supportive measures should be employed.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. Women of childbearing potential should use effective contraception.
