Description
Overview of the medicine
Fix-A-DS 400 mg Capsule contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections including respiratory tract infections, urinary tract infections, otitis media, pharyngitis, tonsillitis, and uncomplicated gonorrhoea.
Uses & Indications
Dosage
Adults
For most infections: 400 mg once daily or 200 mg every 12 hours for 7-14 days. For uncomplicated gonorrhoea: 400 mg as a single dose.
Elderly
No dose adjustment generally required unless severe renal impairment.
Renal_impairment
Creatinine clearance 21-60 mL/min: 300 mg daily. Creatinine clearance <20 mL/min: 200 mg daily.
How to Take
Take the capsule orally with a glass of water, with or without food. It is recommended to take it at approximately the same time each day.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis. This disruption leads to the lysis and death of susceptible bacteria.

Primarily renal (glomerular filtration and tubular secretion) and some biliary excretion.

Absorption
Approximately 40-50% absorbed orally. Food may delay but not significantly reduce absorption.
Metabolism
Not extensively metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase cefixime peak plasma concentration and AUC.
Carbamazepine
Increased carbamazepine levels have been reported.
Warfarin and other anticoagulants
May increase prothrombin time/INR, increasing risk of bleeding. Close monitoring is advised.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy: Category B. Studies in animals have shown no evidence of harm to the fetus. Use in pregnancy only if clearly needed. Lactation: Small amounts of cefixime are excreted in human milk. Caution should be exercised when administered to a nursing mother.
