Fixcef 250 mg Tablet contains Cefixime, a third-generation cephalosporin antibiotic, used to treat a wide range of bacterial infections. It works by preventing the growth of bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg to 400 mg daily, given as a single dose or in two divided doses. For Fixcef 250 mg, commonly 250 mg once daily or 250 mg twice daily depending on infection severity.
Elderly
No specific dose adjustment is required in elderly patients unless there is severe renal impairment.
Renal_impairment
Dosage adjustment is necessary for patients with creatinine clearance less than 60 mL/min. For CrCl 21-60 mL/min, 75% of the standard dose; for CrCl <20 mL/min, 50% of the standard dose.

Fixcef 250 mg Tablet should be taken orally, with or without food. It is recommended to complete the full course of treatment as prescribed by the doctor, even if symptoms improve.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the transpeptidation reaction which cross-links peptidoglycan strands, essential for bacterial cell wall rigidity and strength. This leads to cell lysis and bacterial death.

Onset
Peak plasma concentrations are achieved within 2-6 hours after administration.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. Some excretion also occurs via bile.
Half life
Plasma half-life averages 3-4 hours, potentially longer in elderly patients and those with renal impairment.
Absorption
Approximately 40-50% orally absorbed. Absorption is not significantly altered by food.
Metabolism
Minimal hepatic metabolism; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
Drug Interactions
Probenecid
May increase cefixime plasma concentrations by inhibiting renal excretion.
Carbamazepine
Cefixime may increase carbamazepine levels; therapeutic drug monitoring may be necessary.
Warfarin and other anticoagulants
May increase prothrombin time/INR, leading to an increased risk of bleeding. Close monitoring is recommended.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be supportive.
