Description
Overview of the medicine
Fixcef 500 mg Tablet contains Cefixime, a third-generation cephalosporin antibiotic, used to treat a wide range of bacterial infections including those of the urinary tract, respiratory tract, and skin.
Uses & Indications
Dosage
Adults
The usual adult dose is 500 mg once daily or 250 mg twice daily. Duration of treatment depends on the type and severity of infection, typically 7-14 days.
Elderly
No dosage adjustment is generally required for elderly patients with normal renal function. Use with caution in elderly with impaired renal function.
Renal_impairment
For patients with creatinine clearance less than 60 mL/min, the dose should be reduced. For severe impairment (creatinine clearance <20 mL/min), the dose should not exceed 200 mg once daily.
How to Take
Fixcef 500 mg Tablet should be taken orally, with or without food. It is recommended to complete the full course of treatment as prescribed by the doctor.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
Pharmacokinetics
Onset
Clinical effects usually observed within a few hours of the first dose.
Excretion
Primarily excreted unchanged via the kidney (renal excretion); approximately 50-60% of the absorbed dose is excreted in urine within 24 hours.
Half life
Plasma half-life is typically 3-4 hours.
Absorption
Oral bioavailability is approximately 40-50%. Peak plasma concentrations occur 2-6 hours after administration.
Metabolism
Minimal metabolism; approximately 10% is metabolized.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Warfarin
May increase prothrombin time (INR) and risk of bleeding. Close monitoring of coagulation parameters is advised.
Probenecid
Increases peak plasma concentration and AUC of Cefixime by decreasing its renal clearance.
Carbamazepine
May increase Carbamazepine levels when co-administered with Cefixime.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
