Overview of the medicine
Fixgut 10 mg Tablet contains Domperidone, which is used to relieve nausea, vomiting, and gastric discomfort by enhancing gastrointestinal motility.
Uses & Indications
Dosage
Adults
10 mg three to four times daily, before meals and at bedtime. Maximum 30 mg/day.
Elderly
Similar to adult dosage, but caution with cardiac issues; consider lower dose.
Renal_impairment
Dosage interval may need to be increased to once or twice daily depending on severity. Reduce frequency to once or twice daily for severe impairment.
How to Take
To be taken orally, preferably 15-30 minutes before meals and at bedtime.
Mechanism of Action
Domperidone blocks dopamine D2 receptors primarily in the chemoreceptor trigger zone (CTZ) in the brain and in the gut, thereby reducing nausea and vomiting and promoting gastric emptying.

Typically within 30-60 minutes.
Excretion
About 66% excreted in feces and 33% in urine, largely as metabolites.
Half life
Approximately 7-9 hours in healthy adults.
Absorption
Rapidly absorbed after oral administration; bioavailability is low (around 15%) due to extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver by N-dealkylation and oxidative hydroxylation, primarily by CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to Domperidone or any component.
- •Patients with a prolactin-releasing pituitary tumour (prolactinoma).
- •Moderate to severe hepatic impairment.
- •Conditions where stimulation of gastrointestinal motility might be harmful (e.g., gastrointestinal haemorrhage, mechanical obstruction, perforation).
- •Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, erythromycin).
- •Patients with existing prolongation of cardiac conduction intervals, particularly QTc.
Drug Interactions
Anticholinergics (e.g., Atropine)
May antagonize the prokinetic effect of domperidone.
Dopamine agonists (e.g., Bromocriptine)
Domperidone may antagonize their effects.
Potent CYP3A4 inhibitors (e.g., Ketoconazole, Erythromycin, Clarithromycin)
Increased plasma levels of domperidone, increased risk of QTc prolongation.
Storage
Store below 30°C in a dry place, away from light and moisture.
Overdose
Symptoms include drowsiness, disorientation, extrapyramidal reactions. Management: symptomatic and supportive treatment, gastric lavage, activated charcoal.
Pregnancy & Lactation
Limited data in pregnancy; use only if potential benefit outweighs risk. Excreted in breast milk; advise caution/alternative feeding during lactation.
