Overview of the medicine
Fixotim 200 mg Capsule is an antibiotic used to treat a variety of bacterial infections, including those of the respiratory tract, urinary tract, and ears. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
200-400 mg once daily or 200 mg every 12 hours for 7-14 days depending on the infection.
Elderly
Dosage adjustment not generally required unless severe renal impairment.
Renal_impairment
For CrCl 21-60 mL/min, 200 mg daily; for CrCl <20 mL/min, 100 mg daily.
How to Take
Take orally with or without food. To minimize gastrointestinal upset, it can be taken with food. Do not chew or crush the capsule.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.

Onset
Rapid, typically within 1 hour.
Excretion
Primarily via urine (approx. 50% unchanged) and bile.
Half life
Average 3-4 hours, prolonged in renal impairment.
Absorption
Approximately 40-50% orally absorbed. Food slightly delays absorption but does not affect the extent.
Metabolism
Limited hepatic metabolism.
Side Effects
Contraindications

Drug Interactions
Warfarin
Increased prothrombin time and INR, leading to increased risk of bleeding.
Probenecid
Increases cefixime plasma levels by decreasing renal excretion.
Carbamazepine
Increased carbamazepine levels.
Oral Contraceptives
Reduced efficacy of oral contraceptives (rare).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in low concentrations in breast milk, caution advised during lactation.
