Overview of the medicine
Fixotim 400 mg Capsule is an oral antibiotic containing Cefixime, a third-generation cephalosporin. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg every 12 hours for 7-14 days, depending on the infection. For uncomplicated gonorrhea, a single 400 mg dose.
Elderly
No dosage adjustment needed unless severe renal impairment.
Renal_impairment
Creatinine clearance 21-60 mL/min: 300 mg daily. Creatinine clearance ≤20 mL/min: 200 mg daily.
How to Take
Fixotim capsules can be taken with or without food. To maximize absorption and reduce gastrointestinal upset, it is often recommended to take with food.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Excretion
Primarily excreted unchanged in urine (about 50%) and bile.
Half life
Approximately 3-4 hours
Absorption
Approximately 40-50% orally absorbed. Food can delay but not significantly reduce absorption.
Metabolism
Not extensively metabolized in the liver; limited hepatic metabolism.
Side Effects
Contraindications

- •History of anaphylactic reaction to penicillin
Drug Interactions
Nifedipine
May increase cefixime bioavailability and peak concentration.
Carbamazepine
Concomitant administration may increase carbamazepine plasma concentrations.
Warfarin and other anticoagulants
May increase prothrombin time (PT) and INR, increasing risk of bleeding. Close monitoring is required.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated; otherwise, no specific antidote. Hemodialysis or peritoneal dialysis does not effectively remove cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts into breast milk; use with caution in nursing mothers.
