Description
Overview of the medicine
Fluconazole is an antifungal medication used to treat a variety of fungal infections. It belongs to the azole class of antifungals.
Uses & Indications
Dosage
Adults
Varies by indication: For vaginal candidiasis, a single 150 mg oral dose. For oropharyngeal candidiasis, 50-100 mg once daily for 7-14 days. For systemic candidiasis, 400 mg on day 1, then 200 mg once daily.
Elderly
Dosage adjustments usually not required unless renal impairment is present.
Renal_impairment
Dosage reduction required for creatinine clearance < 50 mL/min. Typically, the initial loading dose remains the same, but subsequent doses are halved or frequency is reduced.
How to Take
May be taken with or without food. Swallow the capsule whole with water.
Mechanism of Action
Fluconazole works by inhibiting the fungal cytochrome P450 enzyme 14α-demethylase, which is essential for ergosterol synthesis in the fungal cell membrane. This leads to increased cellular permeability and leakage of cellular components, ultimately inhibiting fungal growth.
Pharmacokinetics
Onset
Varies by infection, typically within days for symptomatic relief.
Excretion
Primarily renal (approximately 80% as unchanged drug).
Half life
Approximately 30 hours.
Absorption
Well absorbed orally, bioavailability >90%. Peak plasma concentrations achieved within 1-2 hours.
Metabolism
Minor hepatic metabolism (approx 11%), primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to fluconazole or other azole antifungals.
- Co-administration with terfenadine, astemizole, cisapride, pimozide, quinidine, or erythromycin (due to risk of QT prolongation).
Drug Interactions
Warfarin
Increased prothrombin time (INR).
Phenytoin
Increased phenytoin levels.
Rifampicin
Decreased fluconazole levels.
Benzodiazepines (e.g., midazolam)
Increased benzodiazepine levels.
Sulfonylureas (e.g., glibenclamide)
Increased blood glucose lowering effect.
QT-prolonging drugs (e.g., Erythromycin, Quinidine)
Increased risk of arrhythmias.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hallucinations and paranoid behavior. Treatment is symptomatic and supportive, possibly including gastric lavage and forced diuresis. Hemodialysis significantly reduces plasma concentrations.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus, especially for single dose use for vaginal candidiasis. Fluconazole is excreted in breast milk; a single 150 mg dose is generally considered compatible with breastfeeding, but caution is advised for repeated or high doses. Consult a doctor before use.
