Description
Overview of the medicine
Fluconal 150 mg tablet contains Fluconazole, an antifungal medication used to treat a variety of fungal infections, particularly vaginal candidiasis.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: 150 mg as a single oral dose. For other indications, dosage varies; e.g., oropharyngeal candidiasis: 50-100 mg once daily for 7-14 days.
Elderly
Dosage adjustment not usually required unless renal impairment is present. Standard adult dose for normal renal function.
Renal_impairment
Dosage reduction may be necessary based on creatinine clearance (e.g., if CrCl < 50 mL/min, halve the usual dose).
How to Take
Take the tablet orally with or without food. Swallow whole with water.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450-dependent 14α-demethylation of lanosterol, an essential step in fungal ergosterol biosynthesis. This inhibition leads to a depletion of ergosterol and a corresponding accumulation of 14α-methyl sterols in the fungal cell membrane, which disrupts membrane function and inhibits fungal growth.
Pharmacokinetics
Onset
Peak plasma concentrations in 1-2 hours.
Excretion
Primarily renal (about 80% unchanged drug in urine).
Half life
Approximately 30 hours.
Absorption
Well absorbed orally, bioavailability >90%. Peak plasma concentrations are typically reached within 1-2 hours.
Metabolism
Minimally metabolized (approximately 11% by liver); primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to fluconazole, other azole antifungals, or any excipients.
- •Coadministration with drugs known to prolong the QT interval and metabolized by CYP3A4 (e.g., terfenadine, astemizole, cisapride, pimozide, quinidine, erythromycin).
Drug Interactions
Warfarin
Increased prothrombin time (INR); monitor INR closely.
Phenytoin
Increased phenytoin levels; monitor phenytoin concentrations.
Rifampicin
Decreased fluconazole levels; consider increasing fluconazole dose.
Hydrochlorothiazide
Increased plasma concentrations of fluconazole.
Oral Hypoglycemics (sulfonylureas)
Enhanced effect; monitor blood glucose levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include hallucinations and paranoid behavior. Management involves gastric lavage, symptomatic and supportive treatment. Hemodialysis significantly reduces plasma levels.
Pregnancy & Lactation
Generally not recommended during pregnancy, especially in the first trimester, unless the potential benefit outweighs the potential risk. Caution advised during lactation as fluconazole is excreted in breast milk.
