Description
Overview of the medicine
Fluconal 50 mg Suspension contains Fluconazole, a triazole antifungal agent, effective against a wide range of fungal infections, including candidiasis and cryptococcosis. It works by inhibiting the synthesis of ergosterol, a vital component of fungal cell membranes.
Uses & Indications
Dosage
Adults
Varies by indication. For oropharyngeal candidiasis: 50-100 mg once daily for 7-14 days. For esophageal candidiasis: 100-200 mg once daily for 14-30 days. For vaginal candidiasis: 150 mg single oral dose.
Elderly
Similar to adult dosage, but dose adjustment may be necessary for renal impairment.
Renal_impairment
For creatinine clearance <50 mL/min, the dose should be reduced by 50% after the initial loading dose.
How to Take
Fluconal suspension should be administered orally. It can be taken with or without food. Shake the bottle well before each use. Use the provided measuring device for accurate dosing.
Mechanism of Action
Fluconazole selectively inhibits fungal cytochrome P450-dependent 14-alpha-demethylase, which is essential for the conversion of lanosterol to ergosterol. This inhibition leads to a disruption in fungal cell membrane integrity, resulting in fungistatic activity.
Pharmacokinetics
Onset
Clinical effects typically observed within 24-48 hours, but depends on infection type.
Excretion
Mainly renal; approximately 80% of the dose is excreted unchanged in urine.
Half life
Approximately 30 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, with bioavailability >90%. Peak plasma concentrations occur 1-2 hours after administration.
Metabolism
Minimally metabolized; primarily unchanged drug excreted.
Side Effects
Contraindications
- •Hypersensitivity to fluconazole, other azole antifungals, or any excipients of the product.
- •Concomitant use with drugs that prolong the QT interval and are metabolized by CYP3A4, such as terfenadine, cisapride, astemizole, pimozide, and quinidine.
Drug Interactions
Warfarin
Increased prothrombin time (INR); monitor closely.
Phenytoin
Increased phenytoin levels; monitor levels.
Rifampicin
Decreased fluconazole levels; consider increasing fluconazole dose.
Hydrochlorothiazide
Increased fluconazole plasma concentrations; no dose adjustment usually needed.
Cyclosporine, Tacrolimus, Sirolimus
Increased levels of immunosuppressants; monitor levels.
Oral Hypoglycemics (e.g., sulfonylureas)
Increased plasma concentrations; monitor blood glucose.
CYP3A4 substrates (e.g., midazolam, triazolam)
Increased levels of these drugs.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
In case of overdose, supportive care and symptomatic treatment are recommended. Gastric lavage may be considered. Fluconazole is largely excreted in the urine, and forced diuresis would likely increase the elimination rate. A 3-hour hemodialysis session decreases plasma levels by approximately 50%.
Pregnancy & Lactation
Pregnancy Category D (high doses/long-term use) or C (single dose). Use during pregnancy should be avoided unless the potential benefit outweighs the potential risk. Fluconazole is excreted in breast milk; caution is advised during lactation.
