Fluconazole is an antifungal medication used to treat various fungal infections, particularly those caused by Candida species. The 150 mg capsule is commonly prescribed for vaginal candidiasis (yeast infection) as a single oral dose.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: 150 mg single oral dose. For other indications, dosage varies significantly (e.g., 50-400 mg daily for several weeks), determined by the physician.
Elderly
Same as adults, unless significant renal impairment exists.
Renal_impairment
Dosage adjustment is required for patients with creatinine clearance < 50 mL/min; usually, the initial loading dose is the same, then subsequent doses are halved.
How to Take
Oral. Can be taken with or without food.
Mechanism of Action
Fluconazole inhibits the fungal cytochrome P450-dependent enzyme lanosterol 14-alpha-demethylase, which is responsible for the conversion of lanosterol to ergosterol. Ergosterol is an essential component of the fungal cell membrane; its inhibition leads to increased cell permeability, leakage of cellular contents, and eventual cell death.

Side Effects
Contraindications
- •Hypersensitivity to fluconazole or other azole antifungals
- •Co-administration with terfenadine, astemizole, cisapride, pimozide, quinidine, or erythromycin (due to risk of QT prolongation and serious arrhythmias)

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include hallucinations, paranoia, and behavioral changes. In case of overdose, symptomatic treatment and supportive measures should be initiated. Hemodialysis can significantly reduce fluconazole plasma levels.
Pregnancy & Lactation
Pregnancy category D (for long-term high doses), C (for single dose). Not recommended for routine use during pregnancy, especially in the first trimester, unless potential benefits outweigh risks. A single 150 mg dose for vaginal candidiasis is generally considered low risk, but should be used with caution. Excreted into breast milk; caution advised, alternative feeding may be considered for prolonged use.
