Overview of the medicine
Fluconazole is an antifungal medication used to treat various fungal infections, particularly those caused by Candida. The 150 mg tablet is commonly prescribed for vaginal yeast infections.
Uses & Indications
Dosage
Adults
For vaginal candidiasis, a single oral dose of 150 mg. For other indications, dosages vary (e.g., 50-400 mg daily for varying durations).
Elderly
No dosage adjustment usually required unless there is renal impairment.
Renal_impairment
Dosage adjustment needed if creatinine clearance is < 50 mL/min. Initial dose is normal, then subsequent doses are halved.
How to Take
Take the tablet orally, with or without food. Swallow whole with water.
Mechanism of Action
Fluconazole inhibits the fungal cytochrome P450 enzyme 14α-demethylase, which is essential for the synthesis of ergosterol, a key component of the fungal cell membrane. This inhibition leads to a disruption in fungal cell membrane integrity and function, ultimately causing fungal cell death.
Pharmacokinetics
Onset
For vaginal candidiasis, symptomatic relief usually within 24-72 hours.
Excretion
Primarily excreted unchanged in urine (over 80% of the dose).
Half life
Approximately 30 hours, allowing for once-daily dosing or single-dose regimens.
Absorption
Rapidly and almost completely absorbed after oral administration. Bioavailability is over 90%. Peak plasma concentrations are achieved within 0.5-1.5 hours.
Metabolism
Minimally metabolized by the liver (about 11% of dose).
Side Effects
Contraindications
- •Hypersensitivity to fluconazole or any of its excipients.
- •Co-administration with drugs known to prolong the QT interval and metabolized by CYP3A4 (e.g., terfenadine, cisapride).
Drug Interactions
Statins
Increased risk of myopathy and rhabdomyolysis.
Warfarin
Increased prothrombin time.
Phenytoin
Increased phenytoin levels.
Rifampicin
Decreased fluconazole plasma concentrations.
Hydrochlorothiazide
Increased fluconazole plasma concentrations.
Oral Hypoglycemics (sulfonylureas)
Increased plasma concentrations, leading to hypoglycemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hallucinations and paranoid behavior. Treatment is symptomatic and supportive. Gastric lavage may be considered. Hemodialysis significantly reduces plasma levels.
Pregnancy & Lactation
Pregnancy Category D (for prolonged high doses). Single 150 mg dose for vaginal candidiasis is generally considered low risk but should be used only if benefits outweigh risks. Fluconazole is secreted into breast milk. Exercise caution when breastfeeding.
