Overview of the medicine
Fluderm 50 mg Tablet contains Fluconazole, an antifungal medication used to treat a variety of fungal infections, including vaginal yeast infections, oral thrush, and fungal infections of the esophagus, abdomen, lungs, and blood.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: 150 mg single oral dose. For oropharyngeal candidiasis: 50-100 mg once daily for 7-14 days. For systemic candidiasis: 400 mg on day 1, then 200-400 mg once daily.
Elderly
No dosage adjustment needed unless renal impairment is present. Adjust dose based on creatinine clearance.
Renal_impairment
Reduce dose by 50% if creatinine clearance is <50 mL/min. For patients on hemodialysis, give a full dose after each dialysis session.
How to Take
Swallow the tablet whole with a glass of water, with or without food. Take at approximately the same time each day.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450-dependent 14-alpha-demethylation of lanosterol, which is an essential step in fungal ergosterol biosynthesis. This inhibition leads to a depletion of ergosterol in the fungal cell membrane and a buildup of methylated sterol precursors, resulting in increased cell membrane permeability and cell death.

Onset
Clinical effects typically observed within a few days for most indications, though plasma levels peak within 1-2 hours.
Excretion
Primarily excreted unchanged in the urine; about 80% of the dose is excreted unchanged by the kidneys.
Half life
Approximately 30 hours (range 20-50 hours).
Absorption
Well absorbed from the gastrointestinal tract. Oral bioavailability is over 90%.
Metabolism
Minimally metabolized; approximately 11% of the dose is metabolized.
Side Effects
Contraindications

Warfarin
Increased prothrombin time, monitor INR.
Phenytoin

Oral Sulfonylureas
Increased serum concentration, risk of hypoglycemia.
Hydrochlorothiazide
Increased fluconazole levels.
Statins (e.g., Atorvastatin)

Benzodiazepines (e.g., Midazolam)

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hallucinations and paranoid behavior. Treatment involves symptomatic and supportive care; hemodialysis can accelerate fluconazole elimination.
Pregnancy & Lactation
Pregnancy Category D (single dose for vaginal candidiasis may be category C). Use in pregnancy only if potential benefit justifies potential risk. Fluconazole is excreted in breast milk; caution advised during lactation.
