Flunapen 150 mg Capsule contains Fluconazole, an antifungal medication used to treat a variety of fungal and yeast infections. It works by stopping the growth of fungi.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: 150 mg as a single oral dose. For other indications, dosage varies; consult a physician.
Elderly
No dose adjustment is typically required for elderly patients unless there is evidence of renal impairment.
Renal_impairment
Dosage should be reduced by 50% for patients with creatinine clearance less than 50 mL/min, after the initial loading dose.
How to Take
Take the capsule orally with or without food. Swallow whole with a glass of water.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450 enzyme 14α-demethylase, which is necessary for the synthesis of ergosterol, a vital component of the fungal cell membrane. This inhibition leads to the accumulation of 14α-methyl sterols and a decrease in ergosterol, disrupting the fungal cell membrane integrity.

Onset
Clinical effects may be observed within 24-48 hours, though complete resolution of symptoms may take longer.
Excretion
Primarily excreted unchanged by the kidneys (approximately 80% of the dose) via glomerular filtration and active tubular secretion.
Half life
Approximately 30 hours (range 20-50 hours), allowing for once-daily dosing.
Absorption
Well absorbed orally, with an oral bioavailability typically exceeding 90%. Peak plasma concentrations are achieved within 1 to 2 hours post-administration.
Metabolism

Side Effects
Contraindications
- •Known hypersensitivity to Fluconazole or other azole antifungal agents.
- •Concomitant use with terfenadine, cisapride, astemizole, pimozide, or quinidine due to risk of QT prolongation and Torsades de Pointes.
Drug Interactions
Rifampin
Decreased fluconazole plasma concentrations; consider increasing fluconazole dose.
Warfarin
Increased prothrombin time (INR); monitor closely.
Phenytoin
Increased phenytoin serum concentrations; monitor levels.
Oral Sulfonylureas
May cause hypoglycemia; monitor blood glucose.
Hydrochlorothiazide
Increased fluconazole plasma concentrations; consider decreasing fluconazole dose.
Cyclosporine, Tacrolimus
Increased plasma concentrations of immunosuppressants; monitor levels.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive measures and symptomatic treatment should be initiated. Gastric lavage may be considered if recent ingestion. Hemodialysis can significantly reduce plasma fluconazole levels.
Pregnancy & Lactation
Pregnancy Category C (Category D for chronic high doses). Use only if the potential benefit justifies the potential risk to the fetus. Fluconazole is excreted in breast milk; caution is advised when administered to a nursing woman.
