Description
Overview of the medicine
Fluticasone is a synthetic corticosteroid used to reduce inflammation in various conditions, including asthma, chronic obstructive pulmonary disease (COPD), allergic rhinitis, and certain skin conditions. It works by suppressing the immune response and reducing inflammatory mediators.
Uses & Indications
Dosage
Adults
Inhaled (Asthma/COPD): 100-250 mcg twice daily. Nasal (Rhinitis): 1-2 sprays (50-100 mcg) per nostril once daily. Topical: Apply thin layer 1-2 times daily.
Elderly
No specific dosage adjustment usually required, but caution with comorbidities.
Renal_impairment
No specific dosage adjustment required.
How to Take
Inhaled: Administer via MDI or DPI. Shake MDI well before use. Nasal: Prime pump if new or not used for a week. Topical: Apply a thin film to the affected skin area.
Mechanism of Action
Fluticasone binds to glucocorticoid receptors, forming a complex that translocates to the nucleus and modulates gene expression. This leads to the inhibition of inflammatory cell migration and activity, reduction of inflammatory mediators (e.g., cytokines, chemokines, leukotrienes), and vasoconstriction, thereby reducing inflammation and symptoms.
Pharmacokinetics
Onset
Clinical improvement may be seen within 24 hours, but maximum benefit usually takes 1-2 weeks for inhaled/nasal forms.
Excretion
Mainly via feces, with minor renal excretion of metabolites.
Half life
Approximately 7.8 hours (propionate), 24 hours (furoate).
Absorption
Low systemic absorption, especially when inhaled or used nasally, primarily due to first-pass metabolism. Bioavailability is typically less than 30%.
Metabolism
Primarily hepatic, via CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to fluticasone or any component
- •Primary treatment of status asthmaticus or other acute episodes of asthma/COPD requiring intensive measures
Drug Interactions
Ritonavir
Significantly increases systemic exposure to fluticasone, leading to increased risk of systemic corticosteroid effects. Concomitant use is not recommended.
Ketoconazole
May increase systemic exposure to fluticasone; use with caution.
Storage
Store below 30°C. Protect from light, heat, and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose is unlikely to cause acute problems. Chronic overdose may lead to signs of hypercorticism, including adrenal suppression. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C: Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Lactation: Caution should be exercised when administered to a nursing mother.
