Description
Overview of the medicine
Fluticasone Furoate 27.5 mcg Nasal Spray is a corticosteroid medication used to treat symptoms of seasonal and perennial allergic rhinitis, such as sneezing, runny nose, nasal itching, and congestion.
Uses & Indications
Dosage
Adults
Initial dose: 2 sprays (27.5 mcg per spray) into each nostril once daily. Once symptoms are controlled, reduce to 1 spray into each nostril once daily.
Elderly
No dose adjustment required.
Renal_impairment
No dose adjustment required due to low systemic absorption.
Hepatic_impairment
No dose adjustment required for mild to moderate hepatic impairment. Use with caution in severe hepatic impairment.
How to Take
Shake the bottle gently before each use. Prime the pump before the first use or if it has not been used for more than 30 days. Spray directly into the nostril while holding the other nostril closed. Breathe in gently through the nose as you spray.
Mechanism of Action
Fluticasone Furoate is a synthetic trifluorinated corticosteroid with potent anti-inflammatory activity. It works by binding to glucocorticoid receptors in nasal cells, inhibiting the release of inflammatory mediators (e.g., histamine, leukotrienes) and reducing the inflammatory response, thereby alleviating allergy symptoms.
Pharmacokinetics
Onset
Symptomatic relief within 8 hours; full therapeutic effect within several days of regular use.
Excretion
Primarily excreted in feces, mainly as metabolites.
Half life
Plasma terminal half-life is approximately 15 hours after oral administration; specific nasal spray half-life is less relevant due to low systemic absorption.
Absorption
Systemic absorption is low (typically <0.5%) following intranasal administration, with most of the drug being swallowed and then undergoing extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver via cytochrome P450 3A4 (CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to fluticasone furoate or any components of the spray.
Drug Interactions
Cobicistat-containing products
May increase systemic exposure of fluticasone furoate, increasing risk of systemic corticosteroid effects. Avoid concomitant use.
Strong CYP3A4 inhibitors (e.g., Ritonavir, Ketoconazole)
Co-administration may significantly increase systemic fluticasone exposure, potentially leading to Cushing's syndrome or adrenal suppression. Avoid concomitant use.
Storage
Store below 30°C. Do not freeze. Protect from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely with intranasal administration due to low systemic absorption. Chronic excessive use may lead to systemic corticosteroid effects, such as Cushingoid features or adrenal suppression. Treatment is supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is unknown whether fluticasone furoate is excreted in human milk; use with caution in nursing mothers.

Lab Monitoring
- No specific routine laboratory monitoring is required for fluticasone furoate nasal spray due to its low systemic absorption. However, adrenal function tests may be considered if systemic corticosteroid effects are suspected.
Doctor Notes
- Advise patients on correct nasal spray technique to maximize efficacy and minimize side effects.
- Counsel patients that full effect may not be seen for several days.
- Consider systemic corticosteroid interactions, especially with strong CYP3A4 inhibitors.
- Monitor children's growth during prolonged therapy.
Patient Guidelines
- Read the patient leaflet carefully before use.
- Shake the bottle well before each use.
- Use consistently as prescribed for best results, even if symptoms improve.
- Avoid spraying into the eyes.
- Do not exceed the recommended dose.
Missed Dose Advice
If a dose is missed, take the next dose at the usual time. Do not double the dose to compensate for a missed one.
Driving Precautions
Fluticasone Furoate Nasal Spray is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Identify and avoid allergens that trigger your symptoms (e.g., pollen, dust mites, pet dander).
