Description
Overview of the medicine
Frenia 4 mg Tablet contains Olanzapine, an atypical antipsychotic medication used to treat schizophrenia and bipolar disorder. It helps restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 10 mg/day, adjust to 5-20 mg/day. Bipolar mania: Initially 10-15 mg/day, adjust to 5-20 mg/day. Frenia 4mg is suitable for titration or maintenance.
Elderly
Lower starting dose, e.g., 5 mg/day, with careful titration and monitoring, especially if other risk factors are present.
Renal_impairment
No routine dosage adjustment is required for mild to moderate renal impairment, but caution is advised. Severe impairment needs careful consideration.
How to Take
Oral use. Can be taken with or without food. Administer once daily, usually at night.
Mechanism of Action
Olanzapine is an atypical antipsychotic agent that exhibits antagonism at D1, D2, D3, D4 dopamine receptors and 5-HT2A, 5-HT2C, 5-HT3, 5-HT6 serotonin receptors. It also has antagonistic effects at alpha-1 adrenergic, histamine H1, and muscarinic M1, M2, M3, M4, M5 receptors. This diverse receptor binding profile contributes to its broad therapeutic efficacy in psychiatric disorders.
Pharmacokinetics
Onset
Initial antipsychotic effects may be observed within a few days, but full therapeutic effect may take several weeks.
Excretion
Approximately 57% excreted in urine and 30% in feces, predominantly as metabolites.
Half life
Approximately 21-54 hours (mean 33 hours).
Absorption
Well absorbed orally, peak plasma concentrations reached in 5-8 hours. Food does not affect the rate or extent of absorption.
Metabolism
Extensively metabolized in the liver, primarily by direct glucuronidation and by cytochrome P450 mediated oxidation (CYP1A2 and CYP2D6). The major circulating metabolites are inactive.
Side Effects
Contraindications
- Hypersensitivity to olanzapine or any component of the formulation
- Known narrow-angle glaucoma
- Elderly patients with dementia-related psychosis
Drug Interactions
Fluvoxamine
May inhibit olanzapine metabolism, leading to increased olanzapine levels.
Carbamazepine
May increase olanzapine clearance, potentially reducing its efficacy.
Alcohol/CNS depressants
Additive sedative effects; concurrent use should be avoided or used with caution.
Dopamine agonists (e.g., levodopa)
May antagonize the effects of olanzapine.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, blurred vision, rapid heartbeat, agitation, speech difficulties, reduced consciousness, and extrapyramidal symptoms. Management involves supportive care, maintaining airway, monitoring cardiac function, and treating symptoms.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Not recommended during breastfeeding due to excretion in breast milk; a decision should be made whether to discontinue nursing or the drug.
