Uses & Indications
Dosage
Adults
For vaginal candidiasis: 150 mg as a single oral dose. For other indications, dosages vary from 50 mg to 400 mg daily, depending on the infection.
Elderly
No dosage adjustment typically required unless renal function is impaired. Dosage should be adjusted based on creatinine clearance.
Renal_impairment
Initial loading dose of 50-400 mg. Subsequent doses should be reduced by 50% for creatinine clearance < 50 mL/min (e.g., half the usual daily dose).
How to Take
Take orally with or without food. Swallow the capsule whole with water.
Mechanism of Action
Fluconazole works by inhibiting fungal cytochrome P450-dependent 14-alpha-demethylase, an enzyme essential for the synthesis of ergosterol. Ergosterol is a vital component of the fungal cell membrane, and its impaired synthesis leads to increased cellular permeability and cell lysis, thereby inhibiting fungal growth.

Side Effects
Contraindications
- •Hypersensitivity to fluconazole, other azole antifungals, or any component of the formulation.
- •Co-administration with drugs known to prolong the QT interval and metabolized by CYP3A4 (e.g., terfenadine, cisapride, astemizole, pimozide, quinidine, erythromycin) if fluconazole is used at multiple doses or high doses.
Drug Interactions
Warfarin
Increased prothrombin time; monitor INR.
Phenytoin
Increased plasma phenytoin concentrations; monitor phenytoin levels.
Rifampicin
Decreased fluconazole plasma concentrations.
Cyclosporine
Increased plasma cyclosporine concentrations.
Sulfonylureas
Increased plasma concentrations of sulfonylureas; monitor blood glucose.
Hydrochlorothiazide
Increased fluconazole plasma concentrations.
Oral contraceptives
May alter levels of estrogen and progestin.

Overdose
In case of overdose, supportive measures and symptomatic treatment are recommended. Gastric lavage may be considered if recent ingestion. Hemodialysis significantly decreases plasma concentrations of fluconazole.
Pregnancy & Lactation
Pregnancy Category C/D (dose-dependent, for prolonged high doses). Not recommended during pregnancy unless the potential benefit outweighs the potential risk to the fetus. Excreted into breast milk; caution is advised during breastfeeding.
