Description
Overview of the medicine
Funzole 50 mg Suspension is an antifungal medication used to treat a variety of fungal infections, including candidiasis (yeast infections) and cryptococcal meningitis. It contains Fluconazole as its active ingredient.
Uses & Indications
Dosage
Adults
Dosage varies by indication. For oropharyngeal candidiasis, 50 mg once daily for 7-14 days. For systemic candidiasis, 400 mg loading dose on day 1, then 200-400 mg daily. For cryptococcal meningitis, 400 mg on day 1, then 200-400 mg daily for 10-12 weeks after CSF becomes sterile.
Elderly
Dosage should be adjusted based on renal function. No specific dosage adjustment required solely for age if renal function is normal.
Renal_impairment
For creatinine clearance <50 mL/min, the dose should be reduced by 50% after the initial loading dose.
How to Take
For oral use. Shake the suspension well before each use. Can be taken with or without food. Use the provided measuring device for accurate dosing.
Mechanism of Action
Fluconazole inhibits the fungal cytochrome P450-dependent 14-alpha-demethylation of lanosterol, an essential step in fungal ergosterol biosynthesis. This disruption leads to the accumulation of 14-alpha-methyl sterols and depletion of ergosterol, thereby impairing fungal cell membrane synthesis and function.
Pharmacokinetics
Onset
Onset of antifungal action varies depending on the type and severity of infection, typically within days.
Excretion
Primarily renal excretion, with approximately 80% of the dose excreted unchanged in urine.
Half life
Approximately 20-50 hours (average around 30 hours), allowing for once-daily dosing.
Absorption
Well absorbed orally, with bioavailability exceeding 90%. Peak plasma concentrations are reached within 0.5-1.5 hours.
Metabolism
Minimal hepatic metabolism; about 11% of the drug is metabolized.
Side Effects
Contraindications
- Hypersensitivity to fluconazole, other azole antifungals, or any component of the suspension.
- Concomitant administration of terfenadine (with multiple doses of fluconazole) or cisapride due to risk of serious cardiac arrhythmias.
Drug Interactions
Warfarin
Increased prothrombin time, monitor INR.
Phenytoin
Increased phenytoin levels, monitor serum levels.
Rifampicin
Decreased fluconazole plasma concentrations.
Sulfonylureas
Increased risk of hypoglycemia, monitor blood glucose.
Hydrochlorothiazide
Increased fluconazole plasma concentrations.
Cyclosporine, Tacrolimus, Sirolimus
Increased plasma concentrations of these immunosuppressants.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
In case of overdose, supportive measures and symptomatic treatment are recommended. Gastric lavage may be considered if ingestion was recent. Hemodialysis can significantly reduce fluconazole plasma levels.
Pregnancy & Lactation
Pregnancy Category C (high doses can be D). Should be avoided in the first trimester for non-life-threatening infections. Excreted into breast milk; use with caution during lactation, only if potential benefit outweighs risk.

Lab Monitoring
- Liver function tests (LFTs) should be monitored periodically, especially in patients with pre-existing hepatic impairment or during prolonged therapy.
- Renal function tests (e.g., serum creatinine, creatinine clearance) for dose adjustment.
Doctor Notes
- Monitor liver function tests (LFTs) regularly, especially during extended treatment or in patients with pre-existing hepatic impairment.
- Be aware of potential drug interactions, particularly with warfarin, phenytoin, and sulfonylureas. Adjust concomitant medications if necessary.
- Consider renal function for dose adjustments in elderly patients or those with kidney impairment.
Patient Guidelines
- Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
- Shake the suspension well before each use and use the provided measuring spoon/cup.
- Report any unusual or severe side effects, especially yellowing of skin/eyes, dark urine, or severe rash, to your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
