Description
Overview of the medicine
Cefuroxime is a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
750 mg every 8 hours by IM or IV injection. In more severe infections, dosage may be increased to 1.5 g every 8 hours. Max 3-6 g/day.
Elderly
Similar to adults, but dosage adjustment may be necessary in case of renal impairment.
Renal_impairment
Dosage adjustment is required based on creatinine clearance. For CrCl 10-20 mL/min: 750 mg once daily. For CrCl <10 mL/min: 750 mg every 24-48 hours.
How to Take
Furotil 750 mg Injection is administered by deep intramuscular (IM) injection or by slow intravenous (IV) injection over 3 to 5 minutes, or by continuous IV infusion. For IM use, reconstitute with sterile water for injection.
Mechanism of Action
Cefuroxime is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), which interferes with the final transpeptidation step of peptidoglycan synthesis in the bacterial cell wall, thereby inhibiting cell wall biosynthesis and leading to cell lysis.

Rapid, typically within minutes for intravenous (IV) administration.
Excretion
Excreted unchanged in the urine, primarily via glomerular filtration and tubular secretion. Approximately 50% of the dose is excreted within 2 hours.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly absorbed after intramuscular (IM) injection. Peak plasma concentrations are achieved within 30-60 minutes.
Metabolism
Cefuroxime is not metabolized significantly in the body.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime, any other cephalosporin antibiotic, or any of the excipients.
- History of severe hypersensitivity (e.g., anaphylactic reaction) to any other type of beta-lactam antibacterial agent (e.g., penicillins, monobactams, carbapenems).
Drug Interactions
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve (AUC) of cefuroxime by approximately 50%, prolonging its half-life.
Aminoglycosides
Concurrent use of high doses of cephalosporins with aminoglycosides or potent diuretics may increase the risk of nephrotoxicity.
Oral anticoagulants
Cephalosporins, including cefuroxime, may rarely cause prolonged prothrombin time. Caution is advised when cefuroxime is administered concomitantly with oral anticoagulants.
Storage
Store below 25°C to 30°C, protected from light and moisture. Reconstituted solution should be used immediately or stored for a limited time (e.g., 24 hours at 2-8°C or 5 hours at 25°C) depending on specific product instructions.
Overdose
Symptoms of overdose may include cerebral irritation leading to convulsions. Management is primarily supportive and symptomatic. Hemodialysis may be useful in reducing cefuroxime serum levels.
