Description
Overview of the medicine
Gabarol-CR 825 mg is an extended-release tablet containing Gabapentin. It is primarily used to manage postherpetic neuralgia (nerve pain after shingles) and certain types of seizures. The controlled-release formulation allows for once-daily dosing.
Uses & Indications
Dosage
Adults
For Postherpetic Neuralgia: Typically initiated with 300 mg once daily, titrated as tolerated. Gabarol-CR 825 mg is usually taken as two 825 mg tablets (1650 mg total) orally with food once daily in the evening, after dose titration. Maximum recommended daily dose for PHN is 1800 mg (two 825 mg tablets) for some CR formulations.
Elderly
Initiate with lower doses and titrate carefully due to potential for decreased renal function and increased sensitivity to adverse effects. Dosing should be individualized.
Renal_impairment
Dosage adjustment is required based on creatinine clearance. For CrCl <30 mL/min, use is generally not recommended or requires significant dose reduction.
How to Take
Administer orally with food, typically once daily in the evening. Swallow tablets whole; do not split, crush, or chew. Do not take with antacids; separate by at least 2 hours.
Mechanism of Action
The exact mechanism is not fully understood, but Gabapentin is thought to modulate the release of excitatory neurotransmitters by binding to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, thereby reducing neuronal excitability and pain transmission.
Pharmacokinetics
Onset
Onset of action varies. For pain relief, some effects may be noticed within a few days to weeks. For seizure control, stable effects may take longer.
Excretion
Excreted unchanged by the kidneys; dose adjustment required in renal impairment.
Half life
Approximately 5-7 hours. Extended-release forms aim to maintain therapeutic plasma levels over 24 hours.
Absorption
Oral bioavailability is dose-dependent, decreasing with increasing dose. Extended-release formulations are designed for sustained absorption.
Metabolism
Not significantly metabolized in humans; undergoes negligible hepatic metabolism.
Side Effects
Contraindications
- Known hypersensitivity to gabapentin or any component of the formulation
- Acute pancreatitis (rare, but severe cases reported)
Drug Interactions
Opioids
May increase gabapentin levels and increase risk of central nervous system (CNS) depression, including respiratory depression.
Alcohol and other CNS depressants
May potentiate the sedative effects of gabapentin.
Antacids (containing aluminum and magnesium)
Can reduce gabapentin bioavailability; separate administration by at least 2 hours.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, dizziness, diplopia, slurred speech, lethargy, and mild diarrhea. Management is primarily supportive care, including gastric lavage and symptomatic treatment.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Gabapentin is excreted into breast milk; caution advised for nursing mothers. Consult a doctor.

Lab Monitoring
- Renal function should be monitored, especially in elderly or renally impaired patients, and dose adjusted accordingly.
- Monitor for signs of CNS depression and suicidal ideation.
Doctor Notes
- Counsel patients on the importance of not abruptly discontinuing Gabapentin to avoid withdrawal symptoms or exacerbation of seizures.
- Advise patients about potential CNS depressant effects and the risk of suicidal ideation, emphasizing immediate reporting of any mood changes.
- Consider renal function for dose adjustments, especially in the elderly.
Patient Guidelines
- Do not stop taking this medicine abruptly without consulting your doctor, as it may lead to withdrawal symptoms or increased seizure frequency.
- Avoid driving or operating machinery until you know how this medicine affects you, as it may cause dizziness or drowsiness.
- Report any unusual mood changes, depression, or suicidal thoughts to your doctor immediately.
- Take exactly as prescribed by your doctor, preferably with the evening meal.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. Do not double the dose to make up for a missed one. If more than one dose is missed, contact your doctor.
Driving Precautions
Gabarol-CR may cause dizziness, somnolence, or blurred vision. Patients should be advised not to drive or operate hazardous machinery until they are reasonably certain that gabapentin does not adversely affect their ability to engage in such activities.
Lifestyle Advice
- Maintain a balanced diet and regular exercise, as weight gain can be a side effect.
- Avoid alcohol consumption as it may worsen dizziness and drowsiness associated with gabapentin.
