Description
Overview of the medicine
Gemifloxacin is a fluoroquinolone antibiotic used to treat certain bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
320 mg once daily for 5-7 days, depending on the infection.
Elderly
No specific dose adjustment required based on age alone, but renal function should be considered.
Renal_impairment
For creatinine clearance > 40 mL/min: No adjustment. For creatinine clearance ≤ 40 mL/min: 160 mg once daily.
How to Take
Administer orally with or without food. Ensure adequate hydration.
Mechanism of Action
Gemifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Excretion
Approximately 30-40% via urine and 60-70% via feces, largely as unchanged drug.

Approximately 7-10 hours
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 71%. Peak plasma concentrations occur 1-2 hours after oral administration.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Known hypersensitivity to gemifloxacin, other fluoroquinolones, or any component of the formulation.
- •History of tendonitis or tendon rupture associated with fluoroquinolone use.
Drug Interactions
Warfarin
May enhance the anticoagulant effects of warfarin. Monitor INR closely.
QT-prolonging drugs
Increased risk of QT interval prolongation; co-administration should be avoided if possible.
Antacids (containing aluminum or magnesium), Sucralfate, Didanosine (chewable/buffered tablets), Metal Cations (iron, zinc)
Reduce absorption of gemifloxacin; administer gemifloxacin at least 3 hours before or 2 hours after these agents.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
In case of acute overdose, the stomach should be emptied. Observe the patient closely and provide supportive treatment. Hemodialysis and peritoneal dialysis are unlikely to be effective.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Not recommended during lactation as it may excrete into breast milk and cause serious adverse effects in infants.
