Description
Overview of the medicine
Gentob-03 Eye Drop contains Tobramycin, an aminoglycoside antibiotic, used to treat bacterial infections of the eye such as conjunctivitis, keratitis, and other superficial ocular infections.
Uses & Indications
Dosage
Adults
For mild to moderate infections: Instill 1-2 drops into the affected eye(s) every 4 hours. For severe infections: Instill 2 drops into the affected eye(s) hourly until improvement is observed, then reduce frequency.
Elderly
No specific dosage adjustment required for elderly patients, given the minimal systemic absorption.
Renal_impairment
No specific dosage adjustment required for ocular administration due to minimal systemic absorption and renal clearance not being a primary route of elimination for topical use.
How to Take
For ophthalmic use only. Wash hands thoroughly before use. Do not touch the dropper tip to the eye or any other surface to avoid contamination. Tilt head back, pull down the lower eyelid to form a pouch, instill the prescribed number of drops, and then gently close the eye for 1-2 minutes. Press lightly on the inner corner of the eye to prevent the medicine from draining into the tear duct.
Mechanism of Action
Tobramycin works by inhibiting bacterial protein synthesis, primarily by binding to the 30S ribosomal subunit, leading to the death of susceptible bacteria.
Pharmacokinetics
Onset
Rapid onset of action, typically within minutes to hours after instillation.
Excretion
Primarily excreted unchanged in urine if absorbed systemically.
Half life
Not well-defined for ocular application due to minimal systemic absorption; systemic half-life is approximately 2-3 hours.
Absorption
Minimal systemic absorption following ocular administration, resulting in low plasma concentrations.
Metabolism
Not extensively metabolized in the body.
Side Effects
Contraindications
- Hypersensitivity or allergic reaction to tobramycin, any other aminoglycoside antibiotics, or any component of the formulation.
Drug Interactions
Other ophthalmic preparations
If using other eye drops or ointments, allow at least 5-10 minutes between applications to prevent wash-out of the medication. Apply eye ointments last.
Storage
Store at room temperature (below 30°C), away from direct light and moisture. Do not freeze. Keep the bottle tightly closed when not in use. Keep out of reach of children.
Overdose
Ophthalmic overdose is unlikely due to the small volume of the container and minimal systemic absorption. If accidental ingestion occurs, treatment is supportive and symptomatic. Systemic toxicities associated with aminoglycosides (e.g., nephrotoxicity, ototoxicity) are not expected with ophthalmic use.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have not demonstrated a fetal risk, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed and the potential benefit outweighs the potential risk. Tobramycin is excreted in human milk after systemic administration; however, systemic absorption from ophthalmic administration is minimal, so risk to nursing infant is considered low.

YesClinical Trials
Extensive clinical trials have established the efficacy and safety of tobramycin ophthalmic solutions for the treatment of superficial bacterial ocular infections, demonstrating significant microbial eradication rates and clinical improvement.
Lab Monitoring
- No routine lab monitoring (e.g., blood levels) is typically required for ophthalmic use due to negligible systemic absorption.
Doctor Notes
- Instruct patients on proper instillation technique and hygiene to maximize efficacy and minimize contamination.
- Advise patients to contact the clinic if there is no improvement after 3-5 days or if symptoms worsen.
- Be aware of potential cross-sensitivity to other aminoglycosides. If allergic reactions occur, discontinue use immediately.
Patient Guidelines
- Remove contact lenses before applying the eye drops and wait at least 15 minutes before reinserting them.
- Complete the full course of treatment as prescribed by your doctor, even if your symptoms improve to prevent recurrence.
- Avoid sharing eye drops with others to prevent the spread of infection.
- Do not wear eye makeup during treatment.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember, unless it is almost time for your next scheduled dose. In that case, skip the missed dose and continue with your regular dosing schedule. Do not apply a double dose to make up for a missed one.
Driving Precautions
This eye drop may cause temporary blurred vision or other visual disturbances immediately after instillation. If this occurs, avoid driving or operating machinery until your vision is clear.
Lifestyle Advice
- Maintain good hand hygiene, especially before and after applying eye drops.
- Avoid rubbing your eyes excessively, as this can worsen irritation or spread infection.
- Protect your eyes from irritants like dust or smoke.
